 Platforms – Only Six No’s of Platforms [ PLFT-B01, PLFT-B05, PLFT- B10, PLFT – B11,
PLFT-A01, PLFT-A05 ] offered for Inspection after fabrication completion.
 Visual, Inspection and Dimension Inspection of Ladders Carried out. Ladder cage of lower
rungs have not been fabricated, Flat bars for vertical extension not installed.
 Visual, Inspection and Dimension Inspection of Handrails completed on Six nos of
Platforms [PLFT-B01, PLFT-B05, PLFT- B10, PLFT – B11, PLFT-A01, PLFT-A05.] . Rest
not completed. NDT NOT DONE on ALL HANDRAILS.
 Deadman Dimension inspection carried out, Reports not provided.
 Grating clips in Platforms are not falling in place and hence not performing the purpose,
causing possible tripping hazard.

 Fabcon has NOT provided NDE technicians qualification.
 Task no 041 of ITP 1808-145 Revision 5 , welder traceability not maintained by FABCON.
 Fabcon not provided Fitup , Weld visual , NDT reports for Deadman.
 NDT not done on Handrails and remaining Platforms.
 Only Six [PLFT-B01, PLFT-B05, PLFT- B10, PLFT – B11, PLFT-A01, PLFT-A05.] out of
Sixteen Platforms Trial assembly performed.
 Grating Clips in Platform grating do not serve the purpose. Will cause tripping hazard –
(can be Fabcon lack of expertise in installing it)
