What did you learn this week that has contributed to your growth as a nurse? Please explain.

I learned a lot about diabetes from the diabetic nurse and how to manage diabetes specific to the patient’s
needs. I learned about giving the options of the many ways to control diabetes to the patient and giving the
patient the preference of what they would like to do. They could take pills or use short acting insulin or use
long acting insulin.

Which objective did you accomplish this week? How did you do it?

I accomplished participating actively in all clinical experience because I did am care for one of my patient. My
other patient was independent and could do am care on their own. I participated in a consult with the diabetic
nurse. I transferred my patient to the med-surg floor and handed him off to his new nurse. I also participated in
post conference as well

Do you have any concerns about your performance? If so, what are your plans for improvement?
Please explain.

My concern is using the skills I learned in the lab on the floor. I didn’t give any medications or perform any
skills this week but I’m scheduled to give medications next week. My plan is to go into the lab this week and
practice some more.

This week you were able to give me a brief report at the start of the day and verbalize your focus. SBAR was
complete on both patients. Charting was done on both patients and was accurate.
You skipped lunch to stay and hear about diabetic teaching. That is great. I looked for you later and did not
find you on the unit. Did you get a chance to go to lunch? I never had time to ask on Tuesday.
In the future when or if you leave the unit please let me know. You may have been in the bathroom.
I hope you do revisit the skills lab and practice before tomorrow.

Weekly Student Clinical Performance Evaluation

We could not give meds this day for 2 students had not yet given medications at all during this rotation.
We will give meds tomorrow if things go according to my plan. Be prepared at 0830 to discuss the medications
and their effect on the patient. I hope to find an IV and SC insulin in addition to oral medications for us to give.
