Dear Rev. Fr. James Anthony Del Rosario,

Greetings of Peace!

After manifesting their desire to continue fulfilling their ministries and establishing fidelity to the
responsibilities of an Altar Server and exemplary moral standing in the Christian Community as evaluated by a
screening committee, with the privilege granted to me as the coordinator and the approval of our parish priest, I
humbly present to you the Altar Servers of our parish, listed in the attached documents, that are recommended
to partake in the Annual Diocesan Renewal for the Ministry of Altar Servers.

I affirm the knowledge of our ministry about the details of the activity which will be held on November
30, 2019 (Saturday) 7:00 AM – 5:00 PM at Notre Dame of Greater Manila. In the attached document, you
will find the names, ages and the membership statuses of each name listed for recommendation to partake in the
Renewal and Commissioning. Also, together with this recommendation letter is our payment for the registration
amounting to PHP 60.00 for each Altar Server.

Our ministry is with you and the Commission on Worship and the Liturgy in praying for a successful
and meaningful liturgical formation and renewal in our diocese.
