SUMMARY OF INCIDENT

to unload stainless steel sheets from a truck

an injury requiring sutures. The IP’s wound was sutured and dressed.

FINDINGS CORRECTIVE ACTIONS

• The stainless steel sheet
was unloaded manually
instead of using the available
mobile equipment in the area
(Sheets had been loaded
onto the trailer as a “bundle”
using a crane).
• The IP did not anticipate that
the stainless steel sheet
would slip in his hand
injuring his left wrist with the
corner edge of the sheet.

o To strictly implement the use of available lifting
aid (mobile crane or forklift) for loading and
unloading heavy or bulky materials at site.
o At some point manual handling of the sheets
would be required (even if not for unloading).
This incident points to a need to review PPE
(Type of gloves) and to provide longer gauntlet
type gloves to protect the wrist and forearm
areas.
o Given the size of the sheets 4 persons rather than
the 2 on this occasion shall be required when
handling the sheets.
