Dear Sir:

In behalf of __________, I am formally writing your good office to

give notice that the Owner’s Duplicate of the subject title located in
Quezon City consisting of ________ square meters, more or less, is still in
the possession of the _______ as owners thereof. A machine copy of the
title is hereto attached for your reference.

Please be informed that sometime on ______________ and her

husband __________, mortgaged their property covered by the
aforesaid title in favor of Mr. ___________. Corollary, the legal
encumbrance was also annotated on the subject title on ____________.

Considering that the two (2) year period as imposed by Rule 74,
Section 4, of the Rules of Court has already expired without anybody
having presented any claim whatsoever in relation to the said legal
encumbrance, it is submitted that the same may now be canceled so as
to free or release said title of the subject lien or encumbrance.

Hence, we are respectfully requesting your good office to make

the necessary action canceling the annotation on the subject title.

We are hoping for your immediate action and response on the

matter. Thank you.
