Project Proposal

I. Project Title

P-uksain at aksyunan ang

B- ilang ng mga lolo’t lola nating may Dyabetis sa

B- arangay

II. Project Duration

The project will take 5 hours in order to do the screening to the elderly
community people on November 22, 2010at 8 o’clock in the morning until
2 o’clock in the afternoon.

III. Project Scope

The project includes the elderly people within the age group of 60 and
above of Barangay 157 Bagong Barrio, Caloocan City, which will be held at
the barangay health center.

IV. Project Status

The proposed project is new since it focuses on identifying possible


diabetic elderly patients within the community which will be included in
the barangay’s profile on the said disease.

V. Project Beneficiaries

The elderly people of the community can benefit from our project since it
will help them in increasing their awareness about diabetes mellitus. And
in addition to that, some eldrly people from the barangay who are not yet
aware if they have the said disease will be informed regarding what type
of diabetes they have, which could be considered a great help in
preventing the complications and severity of diabetes.

VI. Project Background

Diabetes Mellitus ranked second in the top causes of morbidity in the


Philippines. Through this project, it will help answer the problems or
provide information about the disease, which could aid the community
lessen the number of elderly people that can be affected by diabetes.

VII.Project Justification

The program can help the elderly people to recognize their disease, gain
information and knowledge about their disease in a way they can prevent
further complication. It is necessary to solve the felt problem to help the
elderly people live their life in longer and encourage them to prevent their
disease from becoming severe. Assistance is asked in order to conduct the
program accordingly and disseminate information about the program
evenly.

VIII. Project Objectives

The project aims to identify potential diabetic elderly patients in the


community in helping them with the prevention of the possible
complication of diabetes.

And in order to attain the said general objective the following specific
objectives will be accomplished.

• Identify possible diabetic elderly patients in the community

• Invite the elderly people for community assembly focusing on diabetes


mellitus through health teaching

• Identify and list all the possible elderly diabetic patients through
screening process using the available equipments during the assembly

IX. Project Description

The Health Care Providers committee decided to put up this project in


order to help the elderly people in the barangay be aware of their disease
which is rarely done since it is costly because of the equipments needed.
The committee will invite the elderly barangay people of 157 Bagong
Barrio to attend the screening process in order to indentify how many
elderly people have diabetes in the barangay.

X. Project Proponent

The Health Care providers committee of Group 141 will be in charge to


manage the project together with our clinical instructor who will assist and
guide us in the implementation of the project.

II. Project Duration

The project will take 5 hours to disseminate the leaflets and provide the
health teaching in the barangay on November 22, 2010 at 8 o’clock in the
morning until 2 o’clock in the afternoon.

III. Project Scope

The project includes the elderly people within the age group of 60 and
above of Barangay 157 Bagong Barrio, Caloocan City, which will be held at
the barangay health center.

IV. Project Status

Although leaflet dissemination has already been done by the previous


groups, the content as well as the manner how it will be presented is
completely new.

V. Project Beneficiaries

The beneficiaries of our group will be the elderly people of Barangay 157
especially those who have Diabetes Mellitus.

VI. Project Background

This project will answer the queries of elderly people in the community
about diabetes and the highlight will be on what meals are the best for
diabetic patients.

VII.Project Justification

Inadequate knowledge about the disease is one of the main causes of this
problem. This project can help resolve the problem by giving awareness to
the elderly people on how they can manage their food without risking
their health, since most often times, food is the cause of this type of
disease.

VIII. Project Objectives

The project aims to increase the consciousness of the elderly in the


community about meal planning in order to manage their sugar levels.

IX. Project Description


The project is formed by the Health Care Providers Committee to provide
the most appropriate meal necessary for elderly diabetic patients by
providing information through leaflets, wherein several menus are
presented including few ways on how to cook it. It would be a great help
to the elderly community people and their family members who don’t
have any idea on how to prepare a food for a diabetic patient.

X. Project Proponent

The project will be headed by Health Care Providers Committee of Group
