Informed Consent Form

Name of Project: Celebrity Worship and Social Comparison
You are invited to participate in a study that investigates the celebrity worship syndrome and
social comparison. The study requires you to answer two questionnaires about celebrity worship
and social comparison.

The study is being conducted by Kaye Wennie Rentuma, Alec Rya Sabal, Jirah Cel Telen,
Crishele Mae Tumilap, and Stephanie Valencia, College of Arts and Sciences, Cebu Normal
University (0955-410-0955), telenjirahcel@gmail.com. This research project is undertaken to
meet the requirements of our Research subject under the supervision of Mr. James Louies B.Un
of the Psychology Department.

If you decide to participate, you will be participating in a paper and pen examination. The
preliminary part requires providing some demographic information. The second part consists of
answering two measures regarding celebrity worship and personality. The entire session will take
you no more than 30 minutes to complete. You will be given pens as reimbursement of your
time.

Any information or personal details acquired during the course of the study are confidential
(except as required by law). No individual will be identified in any publication of the results.
Any of your sensitive information will be in absolute discreet to avoid your name from
discrimination. Your security is our utmost concern during the course of this study. Test results
are stored in a locked drawer and access to the data is restricted to the research investigators and
will be solely used for the purpose of this research. After the course of the study, test results will
be shredded to ensure privacy and confidentiality. A summary of the results of the data can be
made available to you on request through email.
Your participation in this study will be of great help to the addition of knowledge to the society
about the personalities linked to the celebrity worshippers nowadays. The study aims to find out
the advantages and problematic areas to the well-being of a celebrity worshipper to give
immediate aid to the worshippers who are in possible danger, and also, to recognize the good
impact brought by worshipping to a celebrity worshipper. Possible risks while participating in
this study may include discomfort while answering the tests provided; sensitive and emotionally-
awakening questions; difficulty in selecting the answers; and the time consumed. Please know
that the time and effort you will render will be converted with exchange compensation. Possible
serious negative effect to you brought by your involvement in this study will be immediately
directed to a psychologist we will be providing.
Participation in this study is entirely voluntary: you are not obliged to participate and if you
decide to participate, you are free to withdraw at any time after taking the first questionnaire and
before taking the second questionnaire, without having to give a reason and without
consequence.

satisfaction. I agree to participate in this research, knowing that I can withdraw from further
