Dear Hon. Balogbog:

Greetings of Peace and Love of Christ!

As stipulated in the Executed Order No. 112, Section 3 of the Guidelines for Areas under General Community Quarantine
(GCQ) including Modified Enhanced Community Quarantine (MECQ) and Modified General Community Quarantine
(MGCQ) which shall observe the following protocol particularly no. 3 “Any person below twenty-one (21) years old, those
who are sixty (60) years old and above, those with immunodeficiency, comorbidities, or other health risks, and pregnant
women shall be required to remain in their residences at all times, EXCEPT when indispensable under the circumstances
for obtaining essential goods and services or for work permitted industries and offices”.

Hence, the school – related transaction is part of the social services. Therefore, this institution adopts the new normal for
the conduct of the aforementioned. As the new school year gets underway it is justly to inform your good office that our
institution scheduled to have a Mobile Enrollment Services for Nursery, Kinder, Preparatory, Grades 7-10 (JHS) and
Grades 11-12 (SHS) on Friday, June 19, 2020 from 8:30am to 12:00noon.

With this, the Mt. Carmel School of Siargao, Inc. has been the living witness of the good governance you have provided to
this barangay and so the undersigned would like to ask approval to the students/pupils from your respective Barangay who
aspire to be part of the Carmelian Community to avail and get Quarantine Pass (if required). It is also with utmost joy on
our part if you could provide and allow us to use any venue for the conduct of the Mobile Enrollment which we can ensure
the safety and well-being of the attendees and at the same time conducive to the preventive measures set by the National
and Local Health agencies. As to our end, we will not entertain nor accommodate clients who will fail to show these passes
including wearing masks and strict observation of physical or social distancing.
