herewith is a machine copy of her NSO Birth Certificate whom my signature can be found
acknowledging her as my daughter as Annex 1;

3. Mr. ___________ wanted to bring her wife and my daughter to his country and to live as
one family;

4. Mr. & Mrs. _______ asked my consent to allow them to bring my daughter with an
assurance that they will support my daughter on her daily needs especially her education and
treat her as their own child;

5. I am giving my CONSENT freely without threat/duress and without reservation to let my

daughter go with her Mother and Mr. ____________ to ___________ and to live there and to
finished her education with the support and care of Mr. & Mrs. ____________ ;  

6. I am executing this affidavit to attest to the veracity of the foregoing facts and for all legal
intents and purpose this may serve
