budget requests.

4/19/2009 Allowed his staff to deface the room sign. This represented a lapse in judgement by Eliseo.
Eliseo’s attitude of begging forgiveness instead of sking permission does not sit well, and his judgement
in this matter was lacking.

12/1/2006 I observed numerous grammatical errors. This is a reoccurring issue with Eliseo. These are
critical skills for a supervisor and he needs to pay more attention to detail. His documentation skills are
poor when it comes to spelling and proper use of grammar. Eliseo has shown very little improvement in
this area. I am constantly reminding him to turn in items into me. This is an unacceptable amount of
time to be late in completing employee evalautions. Eliseo took several days to complete his first
evalaution and averaged less than one evalaution a day.

2/9/2005 I have had issues with paperwork submitted by Eliseo. I have verbalized to him errors with
paperwork that he submits. Eliseo submitted paperwork associated with an administrative inquiry that
had numerous grammatical errors even after he completed it the second time. He failed to complete a
memo that I specifically asked him to complete. At a recent meeting concerning the renovations, Eliseo
had a quote for work from a vendor that was a year and a half old. He could not give an adequate
explanation for what equipment would be needed. In general, I felt that he was not prepared for the
meeting. This was also related to me from other members in the room.
