RE: CONFIRMATION OF SALE

That I, ARCHIBALD A. MENESES, of legal age, Filipino citizen, married, and with postal
address at No. 197 P. Parada St. Brgy. Sta. Lucia, San Juan City, Metro-Manila, after having
been duly sworn to in accordance with law, hereby depose and state, THAT:

1. I am the son of the late, Leopoldo Meneses, registered owner of a motor vehicle more
particularly described as follows:

Plate Number : TVG-540
File Number : 1376-00000115769

2. Sometime in the year 2006, my late father was sold the above-described motor vehicle to
the late EVELIO L. REYES during his lifetime, who was then residing at No. 12 Cristobal St.,
Brgy. Kaunlaran, Cubao, Quezon City;

3. However, due to oversight, my late father failed to retain a copy of the instrument of sale,
for which reason, I presently do not possess a copy of the same;

4. To the best of my knowledge, the aforesaid instrument of sale has not yet been registered
with the Land Transportation Office for purposes of transferring the certificate of registration of
said motor vehicle to Evelio L. Reyes;

5. I hereby affirm and confirm the sale of the above-described motor vehicle to Evelio L.
Reyes, and now that he is already deceased, the same shall properly pertain to his lawful heirs
and successors;

6. I am executing this affidavit to attest to the truth of the foregoing facts, to declare under
oath the valid sale of the above-described motor vehicle in favor of the late Evelio L. Reyes, and
for whatever legal intents and purposes this may serve.
