Name: Acetaminophen (Tylenol)

Drug Class: Non-narcotic analgesic

Action: Decreases fever by (1) a hypothalamic effect leading to


sweating and vasodilation and (2) inhibits the effect of
pyrogens on the hypothalamic heat-regulating centers (3)
inhibits CNS prostaglandin synthesis for analgesic effect (has
NO anti-inflammatory effects)

Purpose of Temporary reduction of fever and relief of minor aches and


Medication: pains due to backache, the common cold, headache,
menstrual cramps, minor arthritis pain, muscular aches, and
toothache.

Safe Dosage: If possible, use client weight to determine the dose, otherwise
use age. A dose of 10 mg/kg has been used for children. Do
not exceed 4 grams/ 24 hr in adults and 75 mg/kg/day in
children. Adults: 325-600 mg q4-6hr of immediate release or
1300mg q6hr of extended release.
Onset: Immediate release – absorbed 30-60 min

Contraindications: Renal insufficiency, anemia, liver failure


Clients with cardiac or pulmonary disease are more susceptible
to acetaminophen toxicity

Side/Adverse CNS: Dizziness, disorientation, and excitement after high


Effects: doses
CV: Methemoglobinemia, hemolytic anemia, neutropenia,
thrombocytopenia, pancytopenia, leukopenia
GI: liver damage (esp after overdose)
Derm: rash/hives, mucosal lesions

Nursing
Considerations

Assess: Prolonged pain/fever


Assess for pain (LOTARP)
Check urine for occult blood and albumin (assess for nephritis)
Determine alcohol use; avoid if >3 drinks/day (causes serious
liver damage)
Use with caution during pregnancy

Administer: Administered orally

Evaluate: Decrease fever


Relief/control of pain

Patient Teaching: Do not combine products containing acetaminophen; dosage is


age and weight determined; take as directed with food or milk
to decrease GI upset; Report paleness, weakness, heart beat
skips, bluish coloration of skin/nailbeds, headache, dizziness,
severe sore throat.
