Please take a moment to complete this dive questionnaire. This questionnaire will help the dive package coordinator determine where
we dive in each port of call. Every diver on the dive boat manifest must complete this Questionnaire and return it by email it to the
Group Leader as soon as possible. Feel free to download this document in WORD and attach it to an email or to cut and paste the info
into the body of the email.
We feel that recent dive experience is more important than certification level; however, if you plan to dive with
the group in Dominica and in St. Lucia, please note that these dive sites may exceed a maximum depth of 100
feet. Advance Open Water Certification and/or 50 logged dives are recommended.
If it has been a while since you last dove (more than three years) please consider taking an inexpensive refresher course
or do a couple of dives locally before the cruise. You will feel more confident in the water and have a more enjoyable
experience. This is only a recommendation. Remember, good air consumption and buoyancy control insures adequate
bottom time for all involved.
If you do not own dive equipment, consider investing in a regulator and dive computer. Divers without computers may
seriously limit the rest of the group’s bottom time. Rental gear is available in all ports of call, but it doesn’t come cheap –
you may pay a lot less to check that extra bag on the plane then it will be to rent gear. Seriously consider taking your own
gear.
