Instructions: Create one issue statement, one long term goal (that describes what it is like when
the identified problem is fixed), one short term goal (that identifies one step to reach the long
term goal) and one intervention. As you do so ensure that you follow the threading principle.
Keep in mind that the intent of this assignment is to practice treatment planning skills, not to
write a full comprehensive treatment plan. Later in the semester you will have the opportunity to
write a full treatment plan. For now, just focus on the treatment planning skills.
Issue Statement for selected issue of concern: (Problem description and barrier)
Tayler is 5 years old and seems to be at the level of a 1 year old. His parents have to get him
ready for the day because he cannot dress, shower, brush his teeth or take care of himself. His
parents feel that it would be great progress if he can learn personal hygiene.
