Materials: 2 copies of sight word identification sheet
Assessment forms – dolch word list assessment sheet - data collection form Velcro Eye gaze board or other method of displaying choices
1. Make 2 copies of the sight word identification sheet.
2. Laminate both copies. Use one copy as the base. Use the other copy to cut out and Velcro onto the base. Put in a binder for easy reference. 3. Print off the data collection form and/or the dolch word list assessment sheet that corresponds to the dolch word list you are working on.
Instructions for the dolch word list assessment sheet:
1. If the child is able to sign or make recognizable verbal attempts, hold up a
word and ask the child what word it is. If they give a correct response put a check mark on the assessment sheet. If the child gets the word wrong or doesn’t know it, place the letter on the eye gaze board or table depending on the child’s ability to make choices. 2. Depending on the child, when you have 2 to 4 letters on the eye gaze board or other method of displaying choices, ask the child to find a certain word. 3. Record the child’s response on the assessment sheet with a dot (this will distinguish it from a verbal or signed response). 4. Repeat the assessment on a regular basis (daily if possible). Using the dolch word assessment sheet, you will be able to see the child’s progress over time. Instructions for the data collection form:
1. Depending on the child, place 2 to 4 words on the eye gaze board or
other method of displaying choices. 2. Ask the child to find a certain word. 3. Accept an eye gaze, point or attempt to reach for a word. You will know your child best and how they make a selection. 4. On the data collection form enter the target word, the choices given (only put 2 if you gave 2 choices) and a correct or incorrect response. It may also be helpful to circle the response chosen in order to analyse the errors. 5. Repeat the same words over several trials to ensure mastery. 6. Repeat with similar words (e.g. words beginning with the same letter, ending with the same letter, etc.) to ensure mastery.
