EE,” the following described premises:

It is further mutually agreed between the LESSOR and the LESSEE the
following:
RENT
Rent shall be payable no later than the day of the month indicated above at the
mailing address of the LESSOR. If rent is not received WITHIN FIVE (5) DAYS of the
day indicated, a service charge of P100.00 PER DAY will be collected for each day
rent is in arrears. Any failure by the LESSEE to pay rent or other charges promptly
when due, or to comply with any other term or condition thereof, shall at the option
of the LESSOR forthwith terminate the tenancy. Any action by the LESSOR shall not
constitute a waiver or modification of this lease unless in writing by the LESSOR.
UTILITIES
DEPOSIT
A security deposit of P_____________, equivalent to one (1) month’s rent, has been
given, for use by the LESSEE as rental on his LAST MONTH OF STAY in the premises,
Provided that: The LESSEE shall inform the LESSOR of such intent, in no less than
thirty (30) days before using the said deposit, or two (2) months before his/her
actual withdrawal from the premises; Provided further that; in case of such failure
by the LESSEE, the deposit given shall be FORFEITED in lieu of damages suffered by
the LESSOR for the abrupt cessation of the contract of lease without notice;
Provided even further: that any premature termination by the LESSEE before the
agreed term of lease shall cause the automatic FORFEITURE of the said deposit
without further ado.
No guest shall be permitted longer than three (3) days within any 30 day period.
LESSEE shall not sublet the demised premises, or any part thereof, or assign this
agreement without the LESSOR’S written consent. Said premises shall be occupied
by no more than ____ persons. LESSEE assumes all legal responsibility of the
acts & conduct of any guests or visitors and assumes financial liability for any
damages done to premises.
If LESSEEs are jointly renting a dwelling the Parties agree that the below
signed LESSEEs shall be JOINTLY AND SEVERALLY LIABLE for the entire rent due
under this lease, so that failure by one LESSEE to pay his share of the rent does not
excuse the other LESSEEs from their obligation to compensate LESSOR for the
entire rent due. LESSEEs further understand that should any one LESSEE breach
any term of this lease, and in particular should any one LESSEE vacate the premises
prior to the expiration of the lease term, that the remaining LESSEEs remain jointly
and severally liable for all damages or rent due for the entire premises.

CARE OF PROPERTY
LESSEE shall keep and maintain the premises in a clean and sanitary
condition at all times, and upon the termination of the tenancy shall surrender the
premises to the LESSOR, clean, undamaged, and empty of all personal possessions.
The LESSOR or his agent shall be privileged to visit, enter, and be upon
said premises, including improvements, at any reasonable hour for the purpose of
either inspecting the same, exhibiting it for sale, rent, or for the purpose of making
repairs and improvements.
LESSEE agrees to refrain from creating unreasonable noise or
disturbances. Should LESSEE’s actions result in the continued disturbance of other
LESSEEs or neighbors, LESSOR will be entitled to serve LESSEE with a Notice of
Breach of this Lease. Should LESSEE receive two (2) such notices from LESSOR and
fail to prevent further disturbances, LESSOR shall have the right to declare LESSEE’s
rights under this Lease forfeited and LESSEE shall be responsible for any rent due or
damages pursuant to the terms of this contract.
LESSEEs agree that they will share and maintain in good condition, the
common areas of the premises. Failure by any one LESSEE to maintain the
common areas in a clean and reasonable condition, and the subjection of the lease
premises for any illegal purpose other than what has been agreed upon by the
parties, may subject that LESSEE to eviction at any time, with prejudice to his/.her
deposit.
LIABILITY
The LESSOR or his agent shall not be held responsible or liable for any damages
or any injury to LESSEE, or his/her property or his/her family, or to his/her family’s
property from whatever cause arising from the occupancy of said premises.
TERM/TERMINATION OF LEASE
LESSEE agrees to vacate the premises within three (3) days after the
termination of the period of lease, or being notified in writing due to non-payment of
rent, or breach of this contract. The parties further agree that should LESSEE
reside/fail to reside within the premises for over three (3) weeks, and further fail
to pay the rent due during that period, or to notify LESSOR in writing of a
forwarding address, LESSOR shall be entitled to consider said action to be as an
abandonment of LESSEE’s interest in the property, and LESSOR shall further be
entitled to enter the premises and arrange for the storage of any personal property
or furniture of the LESSEE, and LESSEE shall be liable to LESSOR for the payment of
any storage charge or incidental expenses. LESSOR aggress to use reasonable care
in moving and storing LESSEEs personal property, and LESSEE hereby holds LESSOR
harmless from all damage caused to property. LESSEE further agrees that if said
property is not picked up by LESSEE within one (1) month of the commencement of
storage of the same, and if the storage and transportation charges are not paid
within one month, the LESSOR may arrange for the reasonable sale of said property,
and may deduct all costs, and expenses from the proceeds.
LESSEE accepts the condition of the property and furnishings at the
commencement of this lease as satisfactory for all purposes of the LESSEE.

ACKNOWLEDGMENT
BEFORE ME, a Notary Public for and in the City of Tuguegarao, Philippines this
_______________ personally appeared _____________ with his Community Tax
Certificate No._____________ issued at ___________ on ___________ known to me and to
me known to be the same person who executed the foregoing instrument and he
acknowledged to me that the same is his own free and voluntary act and deed.
This instrument consists of four (4) pages including this page in which this
acknowledgment is written and duly signed by the LESSOR and LESSEE, together
with his witnesses on each and every page hereof.
