B. Chief Complaints

2c violent to self (self-inflict).͟

C. Client͛s Daily Progress

On February 21, 2011, the patient was admitted at the facility at around 1 p,
accompanied by husband with chief complaints of ͞makulit;slightly violent, nagsisigaw pag
nagagalit, kung sino-sino hinahanap, violent to self (self-inflict).͟ The patient has black eye on
right eye and was put on 4-points restraints due to disturbed behavior, thrown an ice pick to
one of the nursing attendant. The attending physician was informed at 1:16 am, the patient was
referred back to the attending physician due to sleeplessness.
On the next day, February 22, the patient was observed to be argumentative with
shouting bouts and was able to sleep.
The patient was found to be violent and verbally abusive on February 24, and was
transferred to the isolation room.
On February 26 the patient is still in the isolation room, the patient bangs the door with
shouting bouts and was temporarily put on 4-points restraint.
On February 27, the patient is transferred to a female room with shouting bout.
On February 28 to march 1, the client is able to sleep and has a manageable behavior.
