Procedural Checklist
PARENTERAL INJECTIONS

A. Administering Intradermal Injection


Legend:

PROCEDURE: CD ID NT
1. wash hands and observe other appropriate infection control procedures
2. prepare medication from the vial or ampule for drug withdrawal
3. prepare the client.
a. check the client’s identification band
b. ask the client to state his name
4. explain to the client that the medication will produce a small wheal,
sometimes a bleb.
5. provide client privacy
6. select and clean site.
a. select a site on the forearm above the wrist and three to four
finger widths below the antecubital space
b. avoid using sites that are tender, inflamed or swollen and those
that have
lesion
c. put on gloves as indicated by hospital policy
d. cleanse the skin at the site using a firm circular motion at the
center and widening the circle outward
7. prepare the syringe for injection.
a. remove the needle cap while waiting for the antiseptic to dry
b. expel any air bubbles from the syringe
c. grasp the syringe with dominant hand, holding it between thumb
and forefinger
d. hold the needle almost parallel to the skin surface (10-15 degree
angle)
8. inject the fluid.
a. with the non-dominant hand, pull the skin at the site until it is
stretch
b. stabilize the syringe and needle, inject the medication carefully
and slowly so that it produces a small wheal on the skin
c. withdraw the needle quickly at the same angle at which it was
inserted
9. do not massage the injection site
10. dispose of the syringe and needle safely
11. document any relevant information

B. Administering Subcutaneous Medication


Legend:

PROCEDURE: CD ID NT
1. wash hands and observe other appropriate infection control procedures
2. prepare medication from the vial or ampule for drug withdrawal
3. provide client privacy.
4. prepare the client.
a. check the client’s identification band
b. assist the client to position the arm comfortably
5. explain the purpose of the medication and how it will help, using the
language that the client can understand.
6. select the site
7. put on clean gloves
8. clean the site with an antiseptic swab. Start at the center of the site
and clean in a widening circle to about 5 cm (2 in.)
9. place and hold the swab between the third and fourth fingers of the non
dominant hand
10 prepare the syringe for injection
a. remove the needle cap while waiting for the antiseptic to dry. Pull
the cap straight off to avoid contaminating the needle by the outside edge
of the cap
11. inject the medication.
a. grasp the syringe in your dominant hand by holding It between
your thumb and
fingers. With palm facing to the side or upward for a 45 degree angle
insertion.
b. using the nondominant hand, pinch or spread the skin at the site
and insert the needle using the dominant hand and a firm steady push
12. when the needle is inserted, move your nondominant hand to the end of
the plunger.
13. aspirate by pulling back on the plunger. If blood appears in the syringe,
withdraw the needle, discard the syringe and prepare a new injection. If
blood does not appear, continue to administer medication
14. remove the needle slowly and smoothly, pulling along the line of insertion
while depressing the skin with your nondominant hand.
15. dispose of supplies appropriately
16. document relevant information
17. assess the effectiveness of the medication after 30 minutes.

PROCEDURE: CD ID NT
1. wash hands and observe other appropriate infection control procedures
2. prepare the medication from the ampule or vial for drug withdrawal
3. provide for client privacy
4. prepare the client
a. check the client’s identification band
b. asking the patient to state his name
5. assist the client in sitting position comfortably
6. explain the purpose of the medication and how it will help, using language
that the client can understand
7. select the appropiate site.
8. locate the exact site of the injection.
9. put on clean gloves
10. clean site with an antiseptic swab. Using a circular motion, start at the
center and move outward about 5 cm (2 in)
11. transfer and hold the swab between the third and fourth fingers of
your non dominant hand in readiness for needle withdrawal.
12. prepare the syringe for injection.
a. remove the needle cover without contaminating the needle
13. inject the medication
b. hold the syringe between the thumb and forefinger, pierce the
skin quickly and smoothly at a 90 degree angle and insert the needle into
the muscle.
c. hold the barrel of the syringe steady with your nondominant hand
and aspirate by pulling back on the plunger with the dominant hand.
d. aspirate for 5 to 10 seconds. If blood appears on the syringe,
withdraw the needle and discard the syringe.
e. if blood does not appear. Inject the medication steadily and
slowly
14. withdraw the needle at the same angle of insertion
15. apply gentle pressure at the site with a dry sponge. Do not massage the
site.
16. discard the uncapped needle and attached syringe into the proper
receptacle
17. document all relevant information
18. assess the effectiveness of the medication after 30 minutes.
