APPENDIX B

CHECKLIST

APPLICATION TO REGISTER TRAINING PROGRAMME WITH


PEMBANGUNAN SUMBER MANUSIA BERHAD
UNDER THE PROLUS SCHEME

Attached herewith the relevant documents to register the training programmes online with PSMB using
the PSMB Training Provider Information System (TPIS) under the PROLUS Scheme

Training providers under Category A is eligible to register training programme(s) under the PROLUS Scheme.

The documents below are required for attachment in the TPIS :

Bank Payment Slip (Lampiran “F”) of RM300.00 for each training programme payable at
Public Bank Berhad (PBB)

1 set of Appendix B1 for the training programme /course summary which includes:

i. Title of programme / course;


ii. Target group;
iii. Objective of the programme /course; and
iv. Training methodology

1 set of Appendix B3 for the biodata of Trainers


(at least 2 trainers for each training programme complete with their signature)

1 photocopy of the PSMB Train-The-Trainer Certificate / Exemption Letter from


attending PSMB Train-The-Trainer Programme (at least 2 trainers for each training
programme)

One copy of comprehensive training manual. (submit to PSMB by post / hand)

Approval letter to use training manual from

(If training manual is prepared/owed by other parties).

One copy of Training Programme Evaluation Form/Attendance List with at least one run.

Authorisation letter from the Ministry of Education / LAN/ Institution of Higher


Learning from local or overseas for Diploma or Franchise Programme.

Breakdown of training hours and training fee(s) for each module for Training
Programme at Certificate/Diploma Level (Refer to the examples in Appendix B2)

Video tape or other simulation equipments which are used during the training
programme (If used during the session).
