frustration, but through this study, we developed self confidence and more knowledge in
dealing people.

I. Introduction

Protecting and securing the health and wellness of populations using knowledge from
nursing, social and public health sciences. Much of the work is conducted in the community,
through home visits, community clinics and outreach.
Community health nursing is a not only a nursing specialty but also a vocation which
focuses on providing nursing services which benefit community health.Working with individual
patients, community health nurses also target nursing services to specific populations, offering
education, preventive care, and other services to their communities to promote general
wellness and prevention of disease and injury. This career also involves the formulation of
healthcare policy and public health initiatives, all of which are focused on making the
community as healthy as possible.
In a family Health care process, the nurses address the health needs and problems of
their client. It is alogical and systematic way of processing information from the different
sources and translating them into meaningful actions and interventions.Choosing a family to
whom we will render our health services entails many consideratins as to whether this family
would like to cooperate and would give the necessary informations that we need and also the
availability of time of both the nurse and the patient.
Nurses in this field work in government and private agencies, clinics, and other private
settings. They focus on populations, working with individuals, groups, and families to improve
the overall health of communities. They educate about health care issues, disease prevention,
nutrition, and childcare, and also work with community leaders, teachers, parents, and
physicians in community health education.
Family Nursing Care Plan is designed to identify health and Nursing problem in order to
formulate such interventions and solutions of these said problems. The study is particular was
conducted in Twin Peaks, Tuba benguet

A. Source of referral
family was referred by Ms. Cristina Sampayan due to the fact that one of the
members of the family, Mr. has hypertension.

B. Reason of choice of study


I Decided to choose Mr. for a reason that they have a health treat and health
problem that may relate to my FNCP.
C. Expected benefits to be derived from study (student & family)
a. Student
On this study, Benefits not only on the part of the family but also for the students itself
as a reseacher are being facilitated.these benefits on the family will comprise of the
identification of their family problem therefore appropriate actions and interventionsare being
implemented for the total prevention of the problem. And this study for the student being a
researchers serve as training grounds for the future FNCP

b. Family
Asses the health status of the family in the community, for them to identify health
deficit and health threats that pose treat to the health of each family to be visited. To identify
what are the predisposing factors that put the families at risk for requiring diseases and to
promote healthy lifestyle choices through education, public awareness and community
activities.
Developing their skills and knowledge through the community health nirsing to be able
to reach out and share experiences. We emphasized the importance of proper personal
hygiene, proper sanitation, and also for the importance of immunization and vaccinations to
their childrens.

The family is living at Twin Peaks Norte,Tuba Benguet with his two grandsons namely:
, , who are responsible taking care for their grandfather. He is the one who make a decision
for the family .He work as a farmer, though his wife died already his childrens support him,
specially their physiologic needs.The family according to its structure is a nuclear family and
according to its degree of authority, the family is said to be patriarchal since the Grand Father is
the head or the dominant figure of the family. However,in terms of decision-making, especially
in matters of health.

2. Socio- economic & cultural factors

Mr. farmer.Their income depends on what he have been harvesting. Their usual
income is P3,000 which that is not enough for the three of them or is inadequate to meet their
basic commodities such as foods and clothing, but their other relative still supporting their
needs.
They only finish high school. Despite of the various religious sectors in the country, they
are believers under the Baptist church. Though the family live independently, their significant
others or their immediate families or relatives still find time to offer help and support.

3. Home and Environment

The family are originally from Mangkayan, Benguet, but they desided to migrate at Twin
Peaks, Tuba Benguet due to some reasons. As I observed that poor environmental sanitation is
present, because of his age and work he doesn’t enough time to clean up things especially their
garbage disposal that could affect the health status of the family, that may cause certain
diseses like diarrhea, and dengue fever.
Their house is made up of concrete there are one room. They aleady owned the house.
The family have T.V., radio. And they cooked their foods using a wood and gas stove. Their
water came from spring. Open dumping and burning is the type of their garbage disposal.
II. Medical History of the patient/ family

Mr. claimed that they have a history of hypertension in their family. Some of the
common illness occur to the family are cough and colds.He has a BP of 130/90 when we took it
during the home visit. And he said that sometimes it goes up when he is very tired in work.

A. Immunization and Nutritional Status of Children


-Mr. the two of his grandsons completed their immunization.

B. Family Planning
>he does not use any family planning since he is 70 years old and no wife.
C. Health Prctices
In case of health problems of the family, they sometimes do cultural practices before
consulting on a doctor, and in some other problem they consult their relatives and friends.

IV. Word Portrait


According to Mr. he noticed some signs and symptoms of having an high blood
pressure because most of the time she experience headache, dizziness, neck pain, and blurring
of vision after working hard or during the stressful days. And then he decided to go to the clinic
for check up and he discover he has hypertension, and the midwife told him the he need to
take anti hypertensive drugs and avoid straneous activities and some health teaching in
accordance to his condition.

VII. Pathophysiology of the disease (Hypertension)

Common Complications of Hypertension:

The excessive pressure on your artery walls caused by high blood pressure can
damage your blood vessels, as well as organs in your body. The higher your blood
pressure and the longer it goes uncontrolled, the greater the damage.
Uncontrolled high blood pressure can lead to:

 Heart attack or stroke. High blood pressure can cause hardening and thickening of the
arteries (atherosclerosis), which can lead to a heart attack, stroke or other
complications.
 Aneurysm. Increased blood pressure can cause your blood vessels to weaken and bulge,
forming an aneurysm. If an aneurysm ruptures, it can be life-threatening.
 Heart failure. To pump blood against the higher pressure in your vessels, your heart
muscle thickens. Eventually, the thickened muscle may have a hard time pumping
enough blood to meet your body's needs, which can lead to heart failure.
 Weakened and narrowed blood vessels in your kidneys. This can prevent these organs
from functioning normally.
 Thickened, narrowed or torn blood vessels in the eyes. This can result in vision loss.
 Metabolic syndrome. This syndrome is a cluster of disorders of your body's metabolism
— including increased waist circumference, high triglycerides, low high-density
lipoprotein (HDL), or "good," cholesterol, high blood pressure, and high insulin levels. If
you have high blood pressure, you're more likely to have other components of
metabolic syndrome. The more components you have, the greater your risk of
developing diabetes, heart disease or stroke.
 Trouble with memory or understanding. Uncontrolled high blood pressure also may
affect your ability to think, remember and learn. Trouble with memory or understanding
concepts is more common in people who have high blood pressure.

VII. Recommendations for further family care


A. Mr. should always maintain a safe, pleasant and clean environment.
B. Mr. should fully participate and cooperate with the health care team in the
promotion of health and prevention of diseases..
C. Family members should follow the health teaching given to them, especially for
proper sanitation.
D. Mr. should seek medical advice and have a follow up check-up as scheduled.
