• This Payment Voucher (Applicant's Copy)

• PUPCET Payment Fee Receipt of Five Hundred Pesos (Php 500.00) from any LANDBANK branch nationwide

• For high school graduates who have not enrolled in any technical/diploma/degree program immediately after high school graduation, original
or certified true copy if lost, of F138 with school dry seal, and with HSGWA of 82% or higher
• If PEPT qualifier, duly authenticated copy of the Certificate of Rating and the certification issued by the DepEd [eligible for admission to
college with an average rating of 82 or higher in the five subject areas: Mathematics, Communication Arts (English), Science, Communication
Arts (Filipino), and Araling Panlipunan]
• If ALS A&E Test qualifier, duly authenticated copy of the Certificate of Test Result and Completion for Secondary Level [with 100 or higher
Standard Score (SS) and an essay writing proficiency level of 2 or higher]
• School ID (for currently enrolled students) or any valid ID with picture (for previous HS graduates)

PERSONALLY CLAIM YOUR TEST PERMIT THREE (3) WORKING DAYS AFTER PAYMENT TO LANDBANK. Working days are from Monday to
Friday only.

This document is valid only until December 02, 2010.

ALL FEES ARE NONREFUNDABLE AND NONTRANSFERABLE; hence, only those who qualify shall apply. You are allowed to apply and take
the PUPCET in only one (1) campus and only once this school year. Double application will make your PUPCET result null and void.

and correct. My submission of this form constitutes my consent to the collection and use of my information and the transfer of information for processing
and storage by the PUP Web site and the Polytechnic University of the Philippines. Furthermore, I agree and understand that I am legally responsible for
the information I entered in the PUP iApply System, and if I violate its Terms of Service, my information shall be removed and shall lead to the
termination, without prior notice, of my application and everything associated with it, including my personal information.

I also understand that my admission in the University shall be based on passing the PUPCET cut-off score, the interview, and meeting the
University/College requirements on a FIRST-COME-FIRST-SERVED basis, subject to the availability of slots.

Checklist: What to do next?
We have provided you with this checklist on what you are to do next. Follow through these instructions to make
your application official.
Print this document: You need to print this document because you will present this when you pay the
Application Fee and claim your Examination Permit.
Pay the Application Fee: You can pay at any Land Bank of the Philippines (LANDBANK) branch nearest
you. ALL FEES ARE NONREFUNDABLE.

1. Go to the nearest LANDBANK branch nationwide to remit your payment via ONLINE COLLECTION.
2. Fill up three (3) copies of Deposit Slip with the APPLICANT'S FULL NAME (NOT the NAME of
PARENT, GUARDIAN, or any REPRESENTATIVE), Reference Number, PUPCET Account Number,
and Amount to be paid (as indicated in the provided payment voucher). Distribution of Deposit Slip
shall be as follows:
 Original - Accepting Branch
 Duplicate and Triplicate - Payer's Copy
3. Detach the Bank Copy of the provided Payment Voucher.
4. Present to the LANDBANK teller the duly accomplished Deposit Slip with your payment.
5. After processing your payment, the LANDBANK teller will give you the duplicate and triplicate
copies of the Deposit Slip. Keep them together with the Applicant's Copy of the Printed Payment
Voucher.

Please allow three (3) working days after payment to LANDBANK before claiming the
TEST PERMIT.
6. The deadline for payment is on December 02, 2010, 3:00 p.m.

Claim your Test Permit


1. Go to the PUP Admission and Registration Office (ARO) in the specific PUP campus where you applied
for an entrance test in any appropriate attire (with collar and sleeves) for picture taking.
2. Present the following to the Admission Officer:
 Applicant's Copy of the Printed Payment Voucher
 Validated duplicate copy of the LANDBANK Deposit Slip
 Original or certified true copy, if lost, of F138 with school dry seal (for high school graduates who
have not enrolled in any technical/diploma/degree program immediately after high school
graduation)
 Duly authenticated copy of the PEPT/ALS A&E Test Certificate of Rating, certified by the DepEd-
BALS
 School ID (for currently enrolled students) or any valid ID with picture (for previous HS
graduates)

The Admission Officer will validate your application in their records. (Do not forget to sign your
Applicant's Copy of the Printed Payment Voucher)
3. After validation, present yourself for a digital picture taking.
4. The Admission Officer will then issue your printed Test Permit. Before leaving the Admission and
Registration Office (ARO), be sure that the picture and all the data printed in your Test Permit are correct
and clear.

NOTE: Please read all information printed in your Test Permit.


The test permit contains the date, time, place, and any other information about the entrance test. You
will present this permit to the test administrator when you take the entrance test. NO TEST PERMIT, NO
EXAM.

You must also report to the testing room at least 30 minutes before your time scheduled. LATE COMER
EXAMINEES are not allowed to take the test.

Release of PUPCET Result will be announced on the scheduled date of your test. List of Successful
Examinees will be posted thru PUP Web site and in the Bulletin Board at the specific PUP campus where
you took your test.

IMPORTANT ANNOUNCEMENTS to all SUCCESSFUL EXAMINEES will also be posted thru PUP Web site
during the scheduled date of release of PUPCET Result.

Remember: DO NOT misplace these documents.

READ CAREFULLY: Keep the following in a safe place:

 This document
 Your Examination Permit
 Your validated duplicate copy of the LANDBANK Deposit Slip

PUP will not be held responsible for the loss of these documents that will arise from the applicant’s negligence.
NO VALID DOCUMENTS, NO PROCESSING.
