FOREWORD
DET NORSKE VERITAS (DNV) is an autonomous and independent foundation with the objectives of safeguarding life, property and the environment, at sea and onshore. DNV undertakes classification, certification, and other verification and consultancy services relating to quality of ships, offshore units and installations, and onshore industries worldwide, and carries out research in relation to these functions. DNV Offshore Codes consist of a three level hierarchy of documents: Offshore Service Specifications. Provide principles and procedures of DNV classification, certification, verification and consultancy services. Offshore Standards. INTRODUCTION This Service Specification was approved by the Director of Technology in January 2004. This Service Specification enters into force 1 July 2004. The Rules for Classification of Ships Pt.6 Ch. 1 Sec.4 Diving Systems, remain extant until 1 July 2004, after which they will be deleted. The rules for diving systems are contained in two books:

a) The Rules for Classification of Ships Pt.6 Ch.1 Sec.4 (obsolescent) b) The Rules for Certification of Diving Systems 1988 (obsolescent)

SECTION 1 INTRODUCTION
A. Introduction
A 100 General 101 This Offshore Service Specification (OSS) gives criteria for and guidance on certification of complete diving systems, and verification of the integrity of parts of diving systems. 102 Controlling documents relating to offshore systems consist of a three-level hierarchy with these main features:

specified conditions. Other aspects, such as its fitness for purpose, with respect to specified diving operations, may be included in DNVs scope of work, if desired by the client.
A 400 Scope of application for verification 401 This OSS may be adopted for the verification of parts of a diving system. The principles may also be used in communications about the extent and scope of verification activities that are not part of a scheme for certification. A 500 Organisation of DNV-OSS-305 501 This document consists of two sections and six appendices:

Principles and procedures related to DNVs certification and verification services are separate from technical requirements and are presented in DNV Offshore Service Specifications. Offshore Service Specifications present the scope and extent of DNVs services. Technical requirements are issued as self-contained DNV Offshore Standards. Offshore Standards are issued as neutral technical standards to enable their use by national authorities, as international codes and as company or project specifications without reference to DNVs services. Associated product documents are issued as DNV Recommended Practices. The Recommended Practices give DNVs interpretation of safe engineering practice for general use by industry.
Guidance note: Product documents issued under previous document structures may be called Classification Notes or Guidelines. The latest revision of all DNV documents may be found in the publications list in the DNV web site www.dnv.com.

Sec.1 gives the general scope of the document, informative background information, definitions and references. Sec.2 describes the certification process and the activities for each of the project phases. Furthermore, it describes the documents issued during and as a result of the certification process. The use of quality management systems is addressed here too. Appendix A lists the instances where DNV-OS-E402 and DNV-OSS-305 requires certification. Appendix B lists the instances where DNV-OS-E402 and DNV-OSS-305 requires attendance by DNV. Appendix C is a list of sources to assist in obtaining reference documents. Appendix D gives examples of certification documents. Appendix E outlines generic project sub phases. Appendix F is a series of key questions when selecting safety objectives.

describe DNVs certification and verification services for diving systems provide guidance for owners and other parties for the selection of the level of involvement of those carrying out the certification and verification activities provide a common communication platform for describing the extent of verification activities.
A 300 Application 301 This OSS applies to verification, certification and classification during the design, construction and operation of diving systems. Classification is described in more detail in DSVOSS-101 Rules for Classification of Offshore Drilling and Support Units and in DNV Rules for Ships. 302 This OSS describes the necessary activities to be carried out to obtain initial DNV diving system component certificates and DSV class notation. In addition the general requirements outlined in DNV-OSS-101 Rules for Classification of Offshore Drilling and Support Units apply. 303 Statutory certification of diving systems to the requirements of national authorities is not included specifically in the scope of application of this OSS. Such certification shall be governed by the regulations of the appointing authority. However, if these authorities do not give detailed procedures, this OSS will be used by DNV as a guideline for its work when appointed. 304 The primary scope of the certification work is the verification of the components and the integrity of the diving system, and or its capacity to support diving operations under the

B 200 Auxiliary verb forms 201 The terms will, cun and muy are used when describing DNVs actions or activities, and the terms shall, should and muy are used when referring to other parties than DNV. 202 Shall: Indicates requirements strictly to be followed in order to conform with this OSS and from which deviation is normally not permitted. However, deviations may be permitted in the case of technical alternatives as outlined in DNV-OSE402 Sec. 1 A304. 203 Should: Indicates that among several possibilities, one is recommended as particularly suitable, without mentioning or excluding others, or that a certain course of action is preferred but not necessarily required. Other possibilities may be applied subject to agreement. 204 Will: Indicates a mandatory action or activity to be undertaken by DNV. (See shall for other parties.) 205 Can: Indicates an action or activity that DNV not necessarily does unless specifically requested by the Client. (See should for other parties.) 206 Muy: Verbal form used to indicate a course of action permissible within the limits of the OSS. qualified party affirming that, at the time of assessment, the product or service met the stated requirements (BS 4778: Part 2).
Guidance note: For this OSS, the documentis a DNV certificate or a class notation, and it is signed by DNV.

310 Manufacture (DNV-OS-E402): Making of articles or materials, sometimes in larger volumes. In relation to diving systems, refers to activities for the production of pressure vessels, distribution panels and other components, performed under contracts from one or more contractors. 311 Operations @hase): The phase when the diving system is being used for the purpose for which it was designed. 312 Risk (DNV-OS-E402): The qualitative or quantitative likelihood of an accident or unplanned event occurring, considered in conjunction with the potential consequences of such a failure. In quantitative terms, risk is the quantified probability of a defined failure mode times its quantified consequence. 313 Risk reduction measures: Those measures taken to reduce the risks to the operation of the diving system and to the health and safety of personnel associated with it or in its vicinity by:

302 Certification: Used in this document to mean all the verification activities associated with a process leading up to the issue of a Certificate.
Guidance note: In this OSS when Certzjcation is used it designates the overall scope of work or multiple activities for the issue of a Certzjcate, whilst Verzjcation is also used for single activities associated with the work. This in essence means that Certzjcation is Verzjcation for which the deliverable includes the issue of a Certzjcate.

303 Client: DNVs contractual partner. It may be the purchaser, the owner or the contractor. 304 Construction phase (DNV-OS-E402):All phases during construction, including fabrication, installation, testing and commissioning, up until the installation or system is safe and operable for intended use. In relation to diving systems, this includes procurements, manufacture assembly, rectification, installation, testing, commissioning and repair. 305 Design (DNV-OS-E402): All related engineering to design of the diving system. 306 Design phase: An initial phase that takes a systematic approach to the production of specifications, drawings and other documents to ensure that the diving system meets specified requirements (including design reviews to ensure that design output is verified against design input requirements). See IS0 900 1. 307 Hazard: A deviation (departure from the design and operating intention) which could cause damage, injury or other form of loss (Chemical Industries Association HAZOP Guide). 308 HAZOP (HAZard and Operability study): The application of a formal systematic critical examination to the process and engineering intentions of new or existing facilities to assess the hazard potential of inadvertent operation or malfnction of individual items of equipment and their consequential effects on the facility as a whole (Chemical Industries Association HAZOP Guide). 309 Installation (activity) (DNV-OS-E402): The operations related to installing the equipment, diving system or support structure, e.g. mounting chambers and handling systems etc., including fiial testing and preparation for operation.

314 Safety objectives: The safety goals for the construction, operation and decommissioning of the diving system including acceptance criteria for the level ofrisk acceptable to the owner. 315 Statement o compliance: A statement or report signed f by a qualified party affirming that, at the time of assessment, the defined phase, or collection of activities, met the requirements stated by the owner. 316 Verification (DNV-OS-E402): An examination to confirm that an activity, a product or a service is in accordance with specified requirements.
Guidance note: The examination shall be based on information, which can be proved true, based on facts obtained through observation, measurement, test or other means.

SECTION 2 SERVICE OVERVIEW AND REQUIREMENTS


A. General
A 100 Objectives 101 The objectives of this section are to provide:

a) an overview of certification and verification activities relating to diving systems b) details of DNVs certification and verification services for diving systems c) information on the classification process in general.

B. Background (Informative)
B 100 Introduction 101 Certification of diving systems has been carried out historically with a scope and depth of involvement specified by the certif$ng body, with the same end result a certificate. The depth of involvement, or level of certification, has not always been easily detected from the certificate. In addition, the owner may select closer levels of certification involvement to ensure that the certifiing bodys scope is well defined.

the public that the diving system complies with the relevant standard. 204 Additionally, it is good business practice to subject critical work to a third-party check, as this reduces the possibility of errors remaining undetected. Third-party certification will ensure that the verifier has an independent view and perspective when performing this activity. Furthermore, it will avoid the situation where errors could be overlooked by engineers because of their closeness to the work with the diving system. 205 Certification can also be used as a part of the project risk management. As certification increases confidence that the diving system complies with the requirements placed on it by national authorities and the owner, it follows that the risk of failure of the diving system decreases due to independent certification. Thus, certification contributes to project risk management and risk reduction.

C. Overview on Verification, Certification and Classification


C 100 General 101 Applying these principles of verification for distinct smaller or larger parts of the diving system or selected phases may not result in a certificate. Therefore, instead of using the term certification, the term verification is used to describe this service. 102 Verification to DNV-OS-E402, to other standards or to clients specifications may use this OSS as a communication platform and for the preparation of project-specific scope of worktables in the same manner as for certification. C 200 Purpose of verification 201 Verification constitutes a systematic and independent examination of the various phases in the life of a diving system to determine whether it has (or continues to have) sufficient integrity for its purpose. C 300 Verification as a complementary activity 301 Verification shall be complementary to routine design, construction and operations activities und not u substitute for them. Therefore, although verification will take into account the work, and the assurance of that work, carried out by the owner and its contractors, it is inevitable that it will duplicate some work that has been carried out previously by other parties involved in the diving system. C 400 Project management 401 Certification should be planned in close co-operation with the owner and each of its contractors, to provide a scope of work that is tailor-made to the schedule of each production process or activity, i.e. to make the verification activities, surveillance and hold points, an integrated activity.
Guidance note: Some contractors have approved quality control systems and quality control departments, with competent personnel to perform, for example, inspection at manufacturing plants and specialist material engineers competent to review and verify the performance of manufacturers. Reference is made to Manufacturing Survey Arrangements described in DNV Rules for Classificationof Ships Pt.1 Ch.1 Sec.3 A. In that situation DNVs certificationactivities can be confined to:

B 200 Justifications for certification or classification 201 Some national authorities require certification of diving systems and their components and have appointed specific organisations qualified for this work. 202 Some national authorities may have detailed requirements for the certification activity, while others leave the definition of the necessary work-up to the appointed organisation. Guidance note: The purpose of the diving system certificates and class notation is to codirm that the diving system, as installed and ready for use, is in a condition that complies with the technical requirements. It does not codirm that the schedule or cost of the project has been according to plan. Strictly, the class notation covers only the as-installed condition of the diving system and, hence, the conditions affecting this are of particular interest. However, the certification process requires that all the prior phases are verified and that any temporary phases are in accordance with DNV-OS-E402 and has an acceptable risk level. The certification or verification may, in the described scenarios, provide a statement of compliance at the completion of each of the projects stages. Class notation will be provided when the diving system is ready for operation. and only when DNVs scope of work is the certification of the complete proiect.

C 500 Certification to DNV-OS-E402 501 The certification process described in this OSS is tailormade for the certification of diving systems built in accordance with the offshore standard - DNV-OS-E402 Diving Systems. 502 Where DNV-OS-E402 refers to certification, survey, acceptance, agreement or qualification this shall be by DNV. A list of these instances is provided in Appendix B. C 600 Certification to other standards 601 DNV certification to internationally recognised standards shall follow the principles described in this OSS. 602 Where other standards and external criteria are used, the exact terms of reference and documents to be issued shall be agreed at the beginning of the project and formally defiied in the contract. The use of other standards does not allow for a reduction of the quality management requirements as described in the safety philosophy of DNV-OS-E402. 603 DNV reserves the right to call for additional requirements to cover issues essential to the certification and classification process if not covered by the standards in question. 604 It is normally not acceptable to mix standards due to the possible differences in safety philosophies.
Guidance note: Most standards are a coherent collection of requirements for all the relevant aspects of a system. These aspects are normally adjusted to give an overall acceptable safety level. To pick requirements from different standards can then easily result in unpredictable (low) levels of safety. However, in some instances a mixture of standards need to be applied where certain aspects are not covered in one standard alone. (e.g. ASME PVHO-1 applies to acrylic windows regardless of the design code for the pressure vessel.

Guidance note: Where projects or other standards use other terms, these can be used provided they are well defined and agreed. Appendix E includes some generic descriptions of typical subphases. This may be used as guidance when agreeing the content or completeness of a sub-phase.

D 200 Scopes of work 201 Generic scopes of work for certification and verification are given in the tables in this section. 202 The classification will cover the following matters:

a) the vessels ability to keep its position during diving operations b) the hull structural arrangements related to the diving system, e.g. moon pool (launching and recovery well for bell) c) the arrangement and installation of the diving system d) the complete diving system with respect to safety and functioning e) stability and floatability.
D 300 Overall safety objective 301 The owner should define an overall safety objective covering all phases of the diving system from design to operation. The safety objective should address the main safety goals as well as establishing acceptance criteria for the level of risk acceptable to the owner. Depending on the diving operation and its location the risk could be measured in terms of human injuries as well as environmental, political and economic consequences. D 400 Assessment of risk 401 A systematic review should be carried out to identiSl and evaluate the probabilities and consequences of failures in the diving system. The extent of the review shall reflect the criticality of the diving system, the planned operation and previous experience with similar diving systems. This review shall identiSl the risk to the operation of the diving system and to the health and safety of personnel associated with it or in its vicinity. The scope should be agreed upon. 402 Once the risks have been identified their extent can be reduced to a level as low as reasonably practicable by means of one or both of:

D 100 General principles 101 The process of DNVs certification and verification of diving systems is based on distinct project phases and the recognition of key milestones. Verification performed by DNV as part of the certification process, progresses through these project phases and includes all aspects of the project. 102 The certification process follows the project phases: Pre-certification (optional).

602 Factors to be considered when evaluating the adequacy of the quality management system include:

Notification 301 It is assumed that the owner make an assessment of the required safety objectives for a project before preparing tender documents for design and construction activities. This will give contractors clear guidance and reference when estimating the extent and cost of efforts associated with certification activities.

F. Pre-certification
F 100 Verification during conceptual design 101 Verification need not only be performed as part of a complete certification of a diving system but can be standalone service for all or part of a project. 102 Verification during the conceptual and or feasibility studies of a project is not a prerequisite for certification. However, verification of the early stages of a project can reduce the need for verification during the design and construction phases. 103 It is advisable to combine the design verification during pre-certification with additional review of the operational aspects of the support vessel and the project schedule.

G 200 Certification 201 Upon request from the manufacturer, DNV certificates may be issued for a diving system which has been designed, built, equipped, marked, inspected and tested in compliance with the DNV-OSS-305, DNV-OS-E402 and or applicable codes, standards or regulations. These will be stated in the Data Sheet for Diving System (Form 20.201a). 202 The operational limitations and basic assumptions and conditions for use will also be stated in the Data Sheet for Diving System. 203 The Societys commission related to the certification of materials and components (CMC) is completed when the certificates are forwarded to the manufacturer. 204 For environmental control systems a copy of the approved test program completed with the final set points will be endorsed by the Surveyor, and shall eventually be kept available onboard. G 300 Classification 301 Upon class request, the Societys surveyor may inspect the installation of the system and attend the final necessary tests. 302 When a diving system certified by the Society has been installed on a ship or a mobile offshore platform covered by the Societys classification, an additional class notation DSV (see G303) may be included in the ships or the platforms class, provided the following requirements are met:

The requirements for stability and floatability are as given in DNV Rules for Classification of Ships Pt.5 Ch.7 Sec.3 D and Sec.4 B for additional class notation SF, as far as these requirements are applicable. The ship or the platform (semi-submersible) shall be arranged for keeping its position safely during diving operations. The DYNPOS Class Notation AUTR is required as a minimum level of redundancy for diving in conjunction with DP operations. However, contracts may stipulate a higher level of redundancy for certain operations, such as hyperbaric welding. Cranes and other lifting appliances for diving bell handling systems shall be certified in compliance with the Societys rules for Certification of Lifting Appliances (separate publication), as far as applicable, and the additional requirements for handling systems given in DNVOS-E402 Sec.7. The installation and testing requirements given in DNV Rules for Classification of Ships or DNV-OSS-101 Rules for Classification of Offshore Drilling and Support Units shall have been complied with in addition to the testing requirements given in DNV-OS-E402 Sec.2 J, which also apply. The diving system will be subjected to periodical surveys as specified in the respective rules for classification, in order to ensure that the system is properly maintained in good condition for retention of class. In DNV-OS-E402, the major differences between DSV-SURFACE, DSV-BOUNCE and DSV-SAT diving systems appear as requirements for the: a) physical size of the chambers b) life support system c) control stand d) communication system e) capacity of emergency power supply.

Guidance note: As life saving appliances is covered by statutory regulations, there may be overriding requirements for Hyperbaric Evacuation Systems (HES). Consequently, it is important to inform DNV at an early stage what Flag State is intended for the vessel and what geographical areas of operation the diving system should be approved for.

401 It is assumed that overall project management is implemented as a means by which the entire project is controlled. 402 Necessary controls shall be in place to ensure that information flows between the various interfaces. This is especially important where separate contractors have been employed for different phases of the project such as design and installation.

Verification during Design

a) a ship or a mobile offshore platform not covered by the Societys classification, or b) on a fixed offshore installation, or c) on an onshore site, an arrangement similar to classification may be agreed for periodical surveys in order to ensure proper maintenance of the diving system. Corresponding documents may be issued. DNV will require that the ship or mobile offshore platform be classified in a recognized classification society. The main particulars of the diving system will be entered in the Register of vessels classed with DNV. 305 When a diving system, or part of a diving system, has been certified by another recognised classification society, evidence of previous design approval will be required. Such evidence shall include drawings of the arrangement and details bearing the approval stamp, or specifically covered by an approval letter. In addition, for components requiring certification, the corresponding certificates shall be available along with maintenance records. 501 Design verification is the examination of the assumptions, methods and results of the design process and is performed at the specified level of certification to ensure that the specified requirements of the diving system will be achieved. It is recommended that Failure Mode Effect Analysis (FMEA) is carried out concurrently with design.
Guidance note: Levels may apply to various aspects of the certification process, such as the hierarchy of the certificates or the scope of work. The level may be specified by various parties to the contract, by the governing documents or by statutory regulations.

H 300 Validity of Certification Documents 301 Certification documents are, in principle, documents confirming that an examination has been carried out, and are valid only at the date of issue. 302 However, for some certificates a specified period of validiq, and maintenance conditions for ensuring this validity, may be given in the certificate. e.g. statutory and main Class certificates.

Classification Certificate (Class Notation) 401 A DSV Class notation will be issued in the Classification Certificate as a formal statement confirming that the diving system has been completed in accordance with specified requirements. 402 An appendix to the Class certificate and a Data Sheet for diving system will support the Class Notation. 403 The Class notation will only be issued when all relevant activities have been satisfactorily completed. Outstanding issues will prevent the issue of the Class notation unless deemed

of lesser importance, in which case Conditions of Class will be issued. 404 Accompanying documents to the class notation will contain: a) diving system description and item number, in a data sheet (DNV Form 20.201a) accompanying the certificates and reports b) operational limitations and conditions of use for which the diving system is intended c) codes and standards with which the diving system has been found to comply.
405 An officer at DNV H0vik will sign the full-term classification certificate, whereas the local officers will sign the underlying certificates. A local DNV officer will sign the interim class certificate. 406 The DNV H0vik office will file the as-built documentation including the main certificates and drawings. Underlying certificates are filed at the local station. 407 Examples of some documents are shown in Appendix D.
H 500 Verification Comments 501 Reviews of documents will be reported using plan approval letters of response. These letters may give details to the client of aspects that DNV:

702 A survey report will contain enough information to identify clearly the product or service that has been examined, the operating conditions or specifications to which it has been examined and the conclusion reached by DNV. 703 The survey report will be printed on the relevant form and will contain as much information as possible in accordance with the standard headings in the form. In addition, the report number will be shown.

I. Use of Quality Management Systems


I100 General 101 The assurance of diving system integrity requires that gross errors during design, construction and operation be minimised. The likelihood of gross errors shall be reduced in a systematic manner by the operation of a quality management system adequate for the work being carried out. 102 Quality management systems frequently are documented at three levels:

a) the quality manual and related procedures document how the organisation, as a whole, manages the quality of all its products and services b) the quality plan documents the specific procedures related to a particular project c) the inspection and test plan documents how the quality control activities for a particular project shall be carried out and recorded.
I200 Quality Plans 201 The basic function of a quality plan is as an aid in the management of a project. In an organisation with many quality procedures for a variety of functions the quality plan states those that are applicable to that particular project. The quality plan acts as a route map through the complexities of management of the project and highlights those activities relevant to quality management. 202 The project quality plan normally consists of two parts; firstly, a narrative description of the means of controlling the project, and secondly, a tabular description of the inspections and tests to be carried out during the work. 203 The quality plan should address:

a) the company has a documented quality system b) this quality system been certified by an accredited certification body for the product (or service) in question c) the quality system covers the following quality assurance elements adequately for the product: i) organisation ii) authority and responsibility iii) job descriptions for key persons iv) internal quality audits v) documentation change control vi) job instructions and procedures vii) non-conformance and corrective action. d) there are adequate procedures for activities such as: i) calibration of equipment ii) material identification and marking iii) control of special processes such as welding, NDT, PWHT iv) non-conformance identification and handling

204 The narrative part of the quality plan should include a description of:

J100 General 101 During the operations phase, verification in order to maintain the Class notation is outlined in DNV-OSS-101 Rules for Classification of Offshore Drilling and Support Units and DNV Rules for Ships. J 200 Validity of Certification Documents 201 A prerequisite for the retention of validity of certificates in the operations phase is that an in-service contract has been established between the owner, or owners representative, and DNV. 202 The period of validity of a Class certificate subject to examination by DNV will not exceed 5 years. After that period the Class certificate may be renewed, provided a satisfactory renewal survey has been carried out. 203 Some certificates having a period of validity will have a specific area to be signed by DNV annually showing satisfactory completion of the annual assessments required so as to continue the certificates validity. These certificates will become invalid if the signatures are not present. For diving systems this applies to the statutory certificate IMO Diving System Safety Certificate. J 300 Assumptions 301 Classification is based on the assumption that:

I300 Inspection and Test Plans 301 The tabular description of the inspections and tests to be carried out during the work is frequently known as the inspection and test plan. 302 The following items should be checked for inclusion within the inspection and test plan:

a) the diving systems shall be properly maintained and operated by competent personnel b) that a pre-check procedure is followed to ensure that all systems and components function properly before start of each operation c) that current, wave and wind conditions shall be within the design limits for the various systems.
302 Diving systems in operation will retain their class notation provided that:

a) each inspection and test point and its relative location in the production cycle should be shown b) the characteristics to be inspected and tested at each point should be identified c) the use of sub-contractors should be indicated and details of how the verification of sub-contractors quality shall be carried out should be shown d) hold points established by the constructor, the operator or a third party, where witness or review of the selected inspection or test is required, should be shown.
I400 Review of Quality Management Programme 401 The contractors quality manual will be reviewed for compliance with I S 0 9001 or 9002 as appropriate. The contractors operations should be audited to establish compliance with the documented system. 402 If the contractor has a quality system certified by an accredited third party certification body, this may be taken as evidence of a satisfactory quality systemprovided the certificate f is relevant to the contractorS scope o work for the system. However, the last two years periodical audit reports shall be reviewed to identiSl if any recurring non-conformities have been revealed. 403 Any weaknesses revealed during this audit, or review of periodical audit reports, shall be considered when planning the contractor monitoring activities. 404 Surveillance of the continuing acceptability of the contractors quality management system is carried out by observing a selection of audits carried out by the contractor as part of its internal audit system. The audits to be observed should be selected over the length of the project at suitable intervals and

a) the diving system is operated within the specified limitations b) the owner provides adequate documentation from inspection and maintenance activities c) the owner maintains any installed systems for condition monitoring and carries out condition evaluations as applicable d) information about damage, repairs and modifications, which may affect the certification, is promptly reported to DNV e) Conditions of Class issued by DNV are acted upon within the specified time.
303 It is also assumed that whenever a significant component of the diving system is repaired or replaced it is re-tested or certified in accordance with the Service Specification. A report will be issued after any modification or repair of the diving system. 304 Diving systems that are temporarily out of service shall be subject to periodical inspections if the class notation is to be retained. The inspection requirements will be agreed upon between the owner and DNV.

The class notation can be withdrawn if the owner fails comply with the operational procedures for the diving system accepted by DNV carry out the regular in-service inspection and maintenance programme according to the procedures accepted by DNV comply with any conditions of class issued by DNV.

J 500 Obligations 501 To maintain a Certificate during the operational phase, DNV shall be kept informed about the condition of the diving system. DNV shall have full access to information concerning operation, inspection and maintenance activities. 502 The owner shall not@ DNV immediately if any of the events as listed below occur, and DNV will review the need for carrying out assessments and surveys as deemed necessary if:

a) is damaged, or is suspected of having been damaged, in a manner likely to impair its safety or integrity b) demonstrates signs of deterioration likely to impair its safety or integrity c) is subjected to any modifications or repairs, which can impair its safety or integrity d) is considered demobilised.
307 If the situation leading to withdrawal of the Class notation no longer exists, the Notation may be reinstated. However, the diving system will be subject to special assessment or monitoring prior to and or following the reinstating of the Class Notation. J 400 Verification During Operation 401 Verification during operation is carried out by periodical or occasional surveys, of the system in sufficient detail to ensure that the specified requirements of the system continue to be achieved. 402 Assessment of these activities will relate to the owners, as well as any contractors, work. 403 Periodical assessments will be carried out to confirm that any deterioration of the diving system is within acceptable limits and that the diving system continues to be fit for its intended purpose.
Guidance note: Periodic surveys are normally done on an annual basis, with renewal surveys done every fifth year. For DNV class, intermediate surveys are done at the second or third annual survey. (See DNV-OSS-1O1 Rules for Classification of Offshore Drilling and Support Units Ch.3 Sec.4 and DNV Rules for Classification of Ships Pt.7 Ch.2)

a) informed when inspections required to retain the Class notation are to be carried out b) given timely notification of occasions when such parts of the diving system, as are not normally accessible for inspection, may be examined c) given timely advance notification of any diving system repair activities.
504 Once a Condition of Class is issued formally, the validity of the Class is conditional on successful completion of the required action within the specified time. 505 The owner shall notiSl DNV upon the fulfilment of a non-conformance correction, enabling DNV to carry out a verification of the work. 506 DNV is responsible for, in a timely manner:

a) carrying out the agreed assessments b) issuing all reports and any necessary non-conformance report.
J 600 Non-conformance report - Condition of Class 601 Where, after assessment of the diving system during the operations phase, DNV does not consider that items examined meet the specified requirements, non-conformance reports are issued in the form of Conditions of Class. 602 Narrative reports containing all relevant information will be issued. This information shall enable precise identification of the area of DNVs concern. 603 No corrective action will be proposed by DNV, as it is the owners responsibility to do so.

404 Additional assessments shall be carried out to confirm that any damage, deterioration or modification to the diving system does not render the diving system unsuitable for its intended purpose.

APPENDIX E GENERIC DESCRIPTION OF PROJECT SUB-PHASES


A. General
A 100 Introduction 101 Some examples of descriptions of project sub-phases or milestones are given below. These descriptions are not intended to be precise definitions but are given as suggestions as to how particular project-specific definitions can look.

B 100 Feasibility Study 101 A feasibility study is a study directed at evaluation of the feasibility of one or more concepts. 102 The feasibility study is an evaluation of one or more proposed technical concepts against project cost and schedule. It should identi@ special technical problems and indicate the solution or solve these to the degree necessary to confirm the feasibility of the project cost and schedule. 103 The feasibility study shall address all essential cost and schedule aspects, and should conclude on what the most uncertain factors are and how they should be approached. It should preferably address aspects such as support vessel design, special legislation etc. that are peripheral to diving system integrity. B 200 Concept Study 201 A concept study is a design made to establish the main dimensions and data of the diving system. 202 These include, for example, diameter, wall thickness, material type etc. Other aspects include placement on the support vessel and identification of possible needs for major conversion works (existing vessels and designs) and or (additional) surveys. In short, the study has to establish the basic parameters for the work to be performed in the next stages of design and a means for more detailed cost estimation and possibly comparison of a number of concepts detailed to the same level. 203 It is expected only to indicate the preferred or possible methods or solutions of how to solve design issues, configurations, installation practices, etc. On this basis it is expected that the concept study documentation identifies how far the design has reached, what needs to be further detailed or investigated and how the designer foresees the use of the given information as part of the subsequent design.

a) The maximum significant wave height in which the bell is to be launched, the diving system group (SURFACEBOUNCE - SAT), the range of ambient temperatures, maximum operating depth(s), maximum operating time(s), b) number of chambers and their dimensions, number of bells and their dimensions, other pressure vessels and their particulars, gas storage with particulars, c) number of compressors with their particulars, helium reclaim plant with particulars (if fitted), umbilical with particulars, d) basic information on electrical supply and distribution with estimated consumers and capacity, e) proposed fire protection, particulars of the handling system(s) and g) proposed hyperbaric evacuation system. h) A draft to the Data Sheet for Diving System (DNV form 20.201a) should be made to give an overview of the system. This Data Sheet may be updated as the project progresses. In short, to establish a design for which only local details remain to be defined.

APPENDIX F SELECTION OF SAFETY OBJECTIVE


A. General
A 100 General Principles 101 The selection of the safety objective depends on the criticality of each of the elements that have an impact on the management of risks to the diving system. 102 Certification shall direct greatest effort at those elements of the diving system where the risk is highest and whose failure or reduced performance will have the most significant impact on:

a) safety risks b) environmental risks c) economic risks.


103 Suitable selection factors include, but are not limited to, the:

Has this review judged the contribution of each element qualitatively and or quantitatively and used, where possible, quantified risk assessment data to provide a justifiable basis for any decisions made? Does the extent of the review reflect the criticality of the diving system, the planned operation and previous experience with similar diving systems? Does this review identi@ the risk to the operation of the diving system and to the health and safety of personnel associated with it or in its vicinity? Has the extent of the identified risks been reduced to a level as low as reasonably practicable by means of one or both

a) overall safety objectives for the diving system b) assessment of the risks associated with the diving and the measures taken to reduce these risks c) degree of technical innovation in the diving system d) experience of the contractors in carrying out the work e) quality management systems of the owner and its contractors.
104 Due to the diversity of various diving systems, their contents, their degree of innovation, the geographic location, et cetera, it is not possible to give precise guidelines on how to decide what safety objective is appropriate for each particular diving system. 105 Therefore, guidance is given as a series of questions that should be answered when deciding the approprite safety objective for a diving system. This list is not exhaustive and other questions should be added to the list if appropriate for a particular diving system. 106 It must be emphasised that the contribution of each element should be judged qualitatively and or quantitatively. Wherever possible quantified risk assessment data should use to provide a justifiable basis for any decisions made. 107 Depending of the stage of the project, the activities may not have taken place yet in which case the questions can also be posed in another form, i.e. Is . . . . planned to be?

Reduction in the probability of failure? Mitigation of the consequences of failure? Has the result of the systematic review of the risks been measured against the owners safety objective? Has the result of this review been used in the selection of the appropriate certification activity?
