Changes in Police Department Expedited Reporting In the past individuals who reported property damage or incidents of loss or theft

for which there were no witnesses (e.g. stolen cell phone) would dictate the incident to a civilian employee in the 911 center. The call taker would fill out a report form, enter the information into the Police crime data system and assign the incident a case number for insurance claim purposes. Effective May 18, 2011, individuals will be asked to fill out the reports themselves before bringing the completed narrative to the police department to be entered into the police database and assigned a case number. This change will ensure greater accuracy, as the individual who experienced the property damage, loss or theft will be completing the narrative him/herself, and also allow for a more effective use of the Police call takers/Dispatchers time, since they are needed to handle more pressing emergency calls. The revised procedure will be: Citizens will be able to obtain an expedited report form on the Citys web site at www.cityofnewhaven.com/police/forms Forms will also be available at Police Headquarters at One Union Avenue. When the form is filled out, the victim/complainant will bring the form and valid identification to the front desk of the Police Department at One Union Avenue. The Sergeant on duty will verify the information on the form and accept it to be submitted to the PSAP center for entry into the police database. Within 3 days of submission the victim/complainant can call the Police non emergency number 203-946-6316 and obtain a case number for their insurance claim. Any victim/complainant who are unable to complete and deliver the form to the Police Department due to a disability related impairment may call the Citys Department of Services to Persons with Disabilities at 203-946-7651 or TTY/TDD at 203-946-8582 for assistance. Stolen motor vehicles and stolen license plates will still be reported by calling 203-946-6316 immediately so that the information can be entered into the federal and state databases as quickly as possible. This change will help to assure that Dispatchers are not tied up entering reports for insurance claims when more serious emergencies require their attention, and allow the complainant to ensure that the description of the property damage, loss or theft are accurate.

Expedited Reporting Protocol

This form will be available on-line and is to be completed by the victim in cases such as theft from auto, damage to property, or lost and mislaid property; incidents in which the offender is not known, the offender has fled the scene, and, with the exception of auto theft or recovery, when the victim requires a report for insurance purposes. The Expedited Reporting form is pictured below.

Procedure: 1. Victim will download a copy of this form from the city Website. In the event a victim has no Website access, a copy of the form will be available at the front desk of the police department. 2. Victim will complete the information required and produce a valid photo ID to front Desk personnel at 1 Union. (Note: The inability to produce a valid picture ID does not preclude the use of this form). 3. When available, attach and photo copy a picture ID (placed in the appropriate box) on the report and make two (2) copies. 4. One copy is returned to the victim, the other, and the original, are forwarded to PSAP. 5. The victim is instructed to call 946-6316 after three days to obtain the case number 6. PSAP will obtain the case number for the reported incident.
