& BonuTs= Trainee Screening Questionnaire Thank you for your interest in the Do Good Donuts inaugural trainee program! We are so excited to be making this dream a reality and even more excited that you are considering joining us on this adventure. Do Good Donuts is a start-up employment social enterprise that trains youth aged 18 to 28 with intellectual or developmental disabilities for mainstream food service jobs, and pays them while they train, This questionnaire is designed to get to know you a little bit and to help us find the perfect trainees. All we ask is that you answer it honestly! The more honest you are, the more likely it is that we all find success on this journey. There are three sections to this questionnaire. The first section is just so that we can gather some basic information about you. The second section is meant to be answered by the parent/guardian of the potential trainee. The third section is meant to be answered by the trainee themselves (with as little input from parents/guardians as possible).
