Scientific Explanation Nephrotic syndrome is a nonspecific disorder in which the kidneys are damaged, causing them to leak large amounts of protein from the blood into the urine. Damage to the glomeruli causes the membrane to become more porous, so that small proteins, such as albumin, pass through the kidneys into urine.Because of the leaking proteins from the blood to the urine, the nutrients needed in her body are being excreted. This causes her to lose the nutrients in the body, making her nutrition less thanbody requirements
Planning Short term: After 8 hours of nurse- patient interaction, the patient will be able to: Identify the appropriate diet for her condition. (Low-sodium diet) b.Follow the diet prescribed. c.Verbalize realization of the importance of proper
die Long term: After 3days of nursing intervetion the patient will be able to increased of appetite and Vital signs, blood pressure, and laboratory serum studies are within normal range.
Nursing intervention 1.Assess and monitor food/fluid ingested therapy needs -Identifies nutritional deficits/
2.Monitor weight daily at same time, same clothing and same scale. -To assess the health status of patient. Same clothing, same time and same scale makes the weight equal/fair than yesterday.
3.Recommend small, frequent meals. Restrict sodium as indicated, and limit fluid intake to 100ml. -Smaller portions may enhance intake
