I.

Introduction

Community Health Nursing (CHN) is a field of nursing where services are delivered in a community setting. It utilizes the nursing process in different levels of clientele such as individuals, families, population group and communities. The scope of community health nursing encompasses the whole range of health services-----health promotion, preventive, curative and rehabilitative with priority given to keeping people well/healthy and preventing illness among those at risk or exposed to certain health hazards. Keeping the people in the community well and preventing diseases to maintain the highest possible level of community health is a priority in community health nursing practice. This requires planning and implementation of outreach services so that valuable and high risk individuals, families and groups are identified and appropriate health promotion and preventive services are delivered. CHN derives its knowledge and theoretical bases for practice from the Science of Nursing, Public Health, the behavioral Sciences, as well as concepts and principles of Primary Health Care (PHC). Paranaque City with a population of 553, 660 based on 2007 census is one of the cities and municipalities that make up metro manila. It is bordered on the north of Pasay; Taguig to the northeast; Muntinlupa to the southeast; Las Pinas to the southwest and Manila Bay to the west. Paranaque is composed of two congressional districts and two legislative districts which are further subdivided into 16 Barangays. The main focus of the group study is within the community of Silverio Compound, Purok 3. Silverio compound is a mission area with an estimated population of 9,800, and is under the jurisdiction of Barangay San Isidro.

1.1 RATIONALE
Caring is both an art and a science. As an art the nurse utilizes her imagination and innate creativeness in the performance of nursing measure designed to help client cope with health and nursing needs and problems. Silverio Compound is obviously having all forms of problems in the community. Residents are living in congested condition, unhealthy or hazardous environment, and socio-economic status way below the poverty level. As a group we develop and implement community health interventions, measures/ techniques at the individual, family, group and community levels of care. Since the primary focus of care emphasize health promotion and illness prevention, our main focus of care is providing health teaching/ education to individual clients/patients, families and specific population groups during every appropriate opportunity in various settings. For instance, on subjects dealing with health promotion, disease prevention, care of the sick at home, and rehabilitation we can definitely contribute to the health of the community. Moreover, we will also attempt to change individual, family or group health behavior and life style in order to promote and maintain health. For instance, teaching about smoking, nutrition, childcare, and sanitation. Moreover, it is very helpful as a community health care worker to initiate contact, establish and maintain an effective working relationship with individual and people in the community; use of appropriate, valid and reliable methods and tools when assessing the patients.

1.2 Purpose
Data analysis is the process of putting collected data in some kind of logical and systematic order, sorting out the information collected, selecting essential data and discarding nonessential ones. Through assessment and data collection of Silverio Compound residents, the group had identified deficits, problems, difficulties, and individual vulnerability or at risk for health and health related problems. Generally, the people living in the said community are living in poverty. As a community health nurse, it is important to treat people equally, with courtesy and respect. In order for them to feel comfortable and safe, talk to the people in language that they will understand. Also, adopt and demonstrate to them a nonjudgmental attitude. Moreover, as a community health nurses, it is expected that each and everyone has the ability to accurately assess the health status and situation of individual clients, families and specific groups of population based on identified health needs and problems, with the active involvement and maximum participation of the client/patient. Ability to plan the nursing health care, ability to teach, ability to encourage and elicit active involvement and participation of individual patient and families, ability to work harmoniously and cooperatively with others, ability to evaluate the process and outcomes of nursing care.

1.3 Statement of objective


1.3.1 General Objective
Improve condition of the environment.

1.3.2 Specific Objectives

To gather basic information from the community to determine the health concerns and means of the community. To determine the available resources being used by the people in the community in terms of health care. To determine possible factors that affect the health perception of the people and the community. To determine major health care needs and concerns of the residents of the community based upon an analysis of the information gathered such as poverty, employment, unsanitary environmental condition. To develop predictable, measurable community centered goals and outcomes that addresses these identified needs and concerns. To develop a plan of action that includes interventions that will help the residents of the community achieve successful solutions for the identified needs and concerns. To put the plan into action. To evaluate the successfulness of the plan of action.

1.4 Methodology and Tool used


Part of data collection is the inspection of the assigned community environment, its surroundings and the home environment. Data collection that require home visit is a very important method of nursefamily contact. It is through home visits that the nurse will be able to assess the family as a unit---its structure, composition, characteristics, interaction and relationship, and resources. Through home visit, a nurse is able to assess the quality of the home and immediate environment. The nurse will also able to assess the familys ability to perform the health tasks and cope with current health and health-related problems. Grouped in pairs, the group headed down to the assigned community, which is Silverio Compound in Purok 3 with one-person expert to the area to guide us. Each student is required to interview 10 families using a prepared questionnaire on hand then, the whole group then tabulates all the data.
