Congratulations! Your academic achievement qualifies you for National Honor Society consideration. In addition to academic achievement, there are three other qualifiers to meet the membership criteria: Leadership, Service and Character. For consideration, complete the following Student Activity Information application. Upon completion, please print out six copies (double sided, stapled and with this cover sheet, and you can photocopy your picture) and return to Ms. Grace Cheng in the College Counseling Office no later than: 3:35 PM Monday, September 5th, 2011 Late applications will not be considered. The purpose of this form is to give you an opportunity to communicate all of your activities and contributions to TAS as well as to the outside community. It is important that the selection committee have as much information about your activities as possible to make wise decisions. You do not need entries under each category. The categories are there to make it easier for the selection committee to get as full a picture as possible. Here are some general guidelines for filling out this application: With the exception of the short answer questions, keep your responses around 100-200 words. Check off the grade level(s) you in which you have participated in the activity In the column for time, give us some idea of the time commitment of this activity (e.g. one day for 10 hours, an hour a week for 12 weeks, all year for one hour a week, etc.). Feel free to add/subtract rows (but not columns) of the application if you have more activities than spaces provided. Do not forget to get the faculty sponsor signature!
