I. Activities: Taking and recording patients data and history of illness. Monitoring and recording of patients vital signs. Monitoring and regulating of Intravenous fluid according to the desired gtts/min. or cc/hr. Monitoring and recording of patients intake and output every shift. Daily wound care dressing of post-operative surgery patients. Administering of patients medications as ordered by their doctors. Instructed patients about their home medications and follow up check ups upon discharge. Checking and logging of the available medications and supplies in the ward. II. Learning/ Insights: We started in our new area of rotation at Cavite Naval Hospital. We had been oriented about the rules and regulations of the hospital. I was assigned at the surgical ward. The head nurse oriented us about the different cases of patients in the ward and the routine of activities. I became updated and refresh in identifying the patients problem list and the appropriate nursing diagnosis for each problem. Also, the right interventions to be rendered based on the listed problems. III. Issues/ Concerns: I became slight confused in their time format because they based on military time. At first, I still count for the right time especially in giving medications. Another issue is that, I was at ease in the given schedule to us by our head nurse without knowing that the nursing service was not informed about our schedule. The result is that there is a conflict in our assigned schedule. IV. Recommendations: I must adjust myself and be familiarize in the military time and always double check everything for the safety of my patients and also mine. There must be a good relationship and communication among the staff and if in doubt, asking questions and confirmations is the right thing to do to avoid misunderstanding.
