Drug Name of Drug: Diphenhydramine hydrochloride Classification: Antihistamines Anti motion sickness drug Anti parkinsonian Cough suppressant

Sedative-hypnotic

Indications It is used for the symptomatic relief of allergic conditions including urticaria and angioedema. Diphenhydramine is used for its antimuscarinic properties in the control of parkinsonism and drug-induced extrapyramidal disorders

Contraindications Orthostatic hypotension Palpitations Drowsiness Sedation Disturbed coordination

Adverse Side Effects CV & CNS effects. Blood disorders. GI disturbances. Antimuscarinic effects. Allergic reactions.

Nursing Responsibilities Assess respiratory status: rate, rhythm and increase in bronchial secretions wheezing and chest tightness: provide fluids to 2L day to decrease secretion thickness Patient Teaching: Inform patient that drug may cause dry mouth; frequent oral rinses, good oral hygiene and sugarless gum or candy may minimize the effect

Drug Name of Drug: ALBUMIN Classification: Blood product; Plasma protein

Indications Supportive treatment of shock due to burns, trauma, surgery and infections; ARDS: Albumin 25% with a diuretic may be helpful Albumin 25% and loop diuretic may help to control edema

Contraindications contraindicated with hypersensitivity to albumin; severe anemia, cardiac failure, normal or increased intravascular volume, current use of cardiopulmonary b ypass

Adverse Side Effects Hypotension, heart failure, pulmonary edema after rapid infusion Fever, chills, changes in blood pressure, flushing, nausea, vomiting, changes in respiration, rashes

Nursing Responsibilities Consider using whole blood; infusion provides only symptomatic relief hypoproteinemia Monitor BP; discontinue if hypotension occurs Stop infusion if headache, flushing, fever, changes in BP occur, treat with antihistamines Monitor patients clinical response and adjust infusion rate accordingly

Drug Name of Drug: Apidra (Insulin Glulisine) Classification: Insulin preparations Insulin glulisine; Belongs to the class of fast-acting insulins and analogues. Used in the treatment of diabetes.

Indications Treatment of adult patients with diabetes mellitus for the control of hyperglycemia.

Contraindications Hypoglycemia. Special Precautions: Transfer from other types of insulin. Inadequate dosing or discontinuation of treatment. Adjust dose if to undertake increased physical activity or change in usual meal plan. May impair ability to drive or operate machinery. Pregnancy & lactation.

Adverse Side Effects Hypoglycemia, injection site reactions, lipodystrophy, local & systemic hypersensitivity reactions.

Nursing Responsibilities -Should be taken with food (Administer within 15 mins before a meal or within 20 mins after starting a meal.)

Indications Management of acute pain Acute and longterm treatment of signs and symptoms of rheumatoid arthritis and osteoarthritis Reduction of the number of colorectal polyps in familial adenomatous polyposis (FAP)

Contraindications Contraindicated with allergies to sulfonamides, celecoxib, NSAIDs, or aspirin; significant renal impairment; pregnancy; lactation. Use cautiously with impaired hearing, hepatic and cardiovascular conditions

Adverse Side Effects CNS: Headache, dizziness, somnolence, insomnia, fatigue, tiredness, dizziness, tinnitus, ophthamologic effects Dermatologic: Rash, pruritus, sweating, dry mucous membranes, stomatitis GI: Nausea, abdominal pain,dyspepsia, flatulenc e, GI bleed Hematologic: Neutrope nia, eosinophilia, leukopenia, pancytopenia, thrombocytopenia, agranulocytosis, granulocytopenia, aplastic anemia, decreased hemoglobin or hematocrit, bone marrow

Nursing Responsibilities Administer drug with food or after meals if GI upset occurs. Establish safety measures if CNS, visual disturbances occur. Arrange for periodic ophthalmologic examination during long-term therapy. If overdose occurs, institute emergency proceduresgastric lavage, induction of emesis, supportive therapy. Provide further comfort measures to reduce pain (eg positioning,

Adverse Side Effects CV: Thrombophlebitis, vein irritation GI: nausea, vomiting, diarrhea, glossitis, stomatitis, black hairy tongue, enterocolitis, pseu domembranous col itis HEMATOLOGIC: Anemia, thrombocytopenia, thrombocytopenic purpura, eosinophilia, leukopenia, agranulocytosis SKIN: Pain at injection site OTHER: Hypersensitivity re actions, anaphylaxis, overgrowth of non susceptible organisms

Nursing Responsibilities BEFORE: -Assess for hypersensitivity to drug or other penicillin products. -Obtain specimen for culture and sensitivity tests before giving first dose. -Caution for elderly patient having renal problems -Use cautiously in children since their function is not fully developed. DURING: -Monitor liver function test results during therapy, especially in patients with impaired liver functions. Follow the recommended dosage and route of admi nistration as ordered. -Monitor for adverse effects. -Administer the complete course of drug to use the beneficial effects. AFTER: -Advise patient to report rash, fever or chills. A rash is the most common allergic reactions. -Warn patient that IM injection may cause pain at injection site. -Encourage patient to comply with medication regimen. -Protect solution from light and refrigerate ampoules -Instruct patient to promptly report signs of enterocolitis, vomiting, diarrhea and nausea.

Contraindications Contraindicated with hypersensitivity to omeprazole, esomeprazole, or other proton pump inhibitors. -Use cautiously with hepatic dysfunction, pregnancy, and lactation.

Nursing Responsibilities -Assess for hypersensitivity to any proton pump inhibitor or hepatic impairment. -Obtain baseline liver function tests and -Monitor periodically during the course of therapy. -Instruct patient to take drug at least 1 hr before meals. -Instruct patient to report severe headache, worsening of symptoms, fever, chills, darkening of the skin, changes in color of urine or stool. -For GI symptoms such as nausea and vomiting, -Instruct patient to have small, frequent meals. -Instruct patient to limit activities to those that do not require alertness and precision as the drug may cause vertigo and dizziness

Indications For the treatment of hypertension.

Contraindications

Adverse Side Effects back pain;

For patient whose renal function may depend on the activity of the rennin angiotensin aldosterone system (such as those with severe heart failure) treatment with ACE inhibitors and angiotensin receptors antagonist has caused oliguria or progressive azotemia and acute renal failure or death.
