and pain of 9 on 0-10 scale Outcome Criteria/ Client Goal \patient will report pain of 4 on pain scale by 10-20-10 Plan

Alyson Hamilton 10-13-2010 Medical Diagnosis: Bilateral humerus fx Rationale To keep patient free of pain and to allow patient to rest to promote healing Turning and moving patient will decrease pain at site and prevent further complications Opiod analgesics relieve pain and also relieve anxiety which can exacerbate the pain To prevent further pain or dislocation of other limbs To provide patient with proper protein and nutrition to promote healing. To monitor for infection as increased pain may be evidence of infection

Assess: pain on scale of 0-10 prior to med dosing and prn Turn the pt q2hrs and reposition for comfort Administer Morphine as prescribed by physician as ordered and prn Handle patients body with care when repositioning Schedule consult with dietician prn

Evaluation: Goal not met. Pt continue to report pain of 7 on pain scale 10-20-10

Teach client to report increased pain in affected area
