GFI Application Form TERMS AND CONDITIONS

I am not an employee, agent or legal representative of GF International (hereinafter referred to as the Company, GlobalFusion or GFI) or other affiliated GlobalFusion companies. I am an independent contractor of legal age (at least 18 years of age), who agrees to be responsible for learning and complying with all applicable and relevant laws and to pay any taxes and other assessments required by national, state, municipal and local laws or other statutes and regulations. I understand that I can delegate obligations under the Contract to others, but that I am solely responsible for ensuring that anyone to whom I delegate fully complies with these terms and conditions. I confirm that I will not use or appropriate to myself, directly or indirectly, the registered or unregistered trademarks, trade names and other proprietary marks of the Company or any of the affiliated companies thereof (Trademarks"), whether or not used in commerce, to promote its business opportunity or to register the said Trademarks as domain names without prior written approval from GlobalFusion. I agree that any unauthorized use of the Trademarks constitutes a material breach of this agreement and that all rnaterials and products bearing the unauthorized Trademarks or domain names in any registration or application thereof shall automatically become property of GlobalFusion or its affiliated companies and must be assigned unconditionally to GFI.

I have been presented to and I understand the Marketing & Compensation Plan as well as the Policies and Procedures for GlobalFusion Independent Distributors (the "Policies and Procedures"). I agree and abide by the terms and conditions stated herein stated in these documents.

I agree that the terms and conditions of the Marketing & Compensation Plan and the Policies & Procedures may be changed and updated from time to time to include revisions, supplements and amendments and these changes thereto be automatically incorporated herein by this reference. 11. This is an integrated Contract (the "Contract") that also includes these documents. No other promises, representations, guarantees or agreements of any kind shall be valid unless in writing and signed between GlobalFusion and myself. 12. I acknowledge that any violation of this Contract or any other agreements or obligations I may have with GlobalFusion or any of its affiliated entities may result in the termination of my membership or in another disciplinary action as deemed appropriate by GlobalFusion. 13. This Contract allows me to sponsor prospective Distributors of GlobalFusion. I may sponsor distributors in other places and countries pursuant to the conditions of the GlobalFusion Expansion Program. 14. I understand, agree and confirm that I shall abide by all the medical protocol as instituted by GlobalFusion Incorporated. 15. I certify to the accuracy of all information provided by me in this Application Form and agree that providing false or misleading information authorizes GlobalFusion, at its election, to declare this Contract void from its inception. 16. I understand that only Distributors in good standing (such status is determined by GlobalFusion) may sponsor new Distributors. GlobalFusion, in its sole discretion, may reject this contract without disclosing any reason thereof. 17. If this Application form is not accepted or approved, I release GlobalFusion and its officers, directors, agents, advisors, and employees from all liability incurred by me or by any other person. I waive any associated claim(s) that might be asserted in my interest.

I agree that I will not make any statements, representations, claims, or warranties regarding GlobalFusion products which are not contained in official GlobalFusion promotional materials produced and distributed by GlobalFusion of which are otherwise authorized by an affiliated GlobalFusion company for distributorship in my area. I understand that a purchase of one of the GFI packages is necessary to become an Independent Distributor of GFI. I agree that as an Independent Distributor, I shall enjoy a 30% discount from the SRP of all GFI products. I will become an independent contractor and distributor of GlobalFusion and I will, at all times, have the right to sell GlobalFusion products (the "Products") in accordance to the Marketing & Compensation Plan of GFI (the "Compensation Plan") This application form will be deemed accepted by GlobalFusion when it is entered into the GlobalFusion database. The application form must be submitted personally at the business centres designated by GlobalFusion and must be stamped receipt by authorized GlobalFusion personnel.
