 Name of the facility: "Jesús Redentor" Community Mental Health Center.

 Location: Nuevo Sullana/Sullana - Piura

 Unit: Adult and Older Adults

 Population Sullana: 317,4431 inhabitants

 New Sullana population: 16152 inhabitants

GENERAL OBJECTIVE

 To provide professional care through prevention, intervention and treatment at


the JESUS REDEEMER COMMUNITY MENTAL HEALTH CENTER, Adult
and Elderly Unit.

SPECIFIC OBJECTIVES

 Specialized outpatient care for the adult and elderly population.


 Interdisciplinary management in psychosocial rehabilitation.
 Interdisciplinary management of the Continuity of Care Program (PCC) ).
 Clinical accompaniment to mental health facilities.
 Development of two specific assistance programs:
a) Psychosis and

b) Violence.

 Development of strategies for the maintenance of care for patients belonging to


the Adult and Elderly unit.
 Participation in training processes by the health subregion.
 Articulate mental health care with community institutions and organizations.
 Develop programs for community participation in civic dates.
 To develop research activities in mental health and/or teaching.
 Encourage the care of the community mental health center's own
multidisciplinary team.

ACTIVITIES TO BE PERFORMED

 Design the therapeutic framework in a specialized way for each patient.

 Scheduled individual patient care in coordination with the interdisciplinary team.


 Unit meetings to review patient follow-up and treatment.

 Maintain the constant updating of the follow-up registry for the specialized

treatment of each patient.

 Elaboration of informative material related to violence.

 Implementation of the program against family and gender violence.

 Implementation of the program for people with psychotic symptoms.

 Perform patient monitoring through calls.

 Conduct home visits to patients for the positive development of their

intervention.

 Execution of training sessions for health professionals.

 To measure the effect of an individual recovery program, using solution-focused

techniques, for people affected by violence.

 Execution of integration and motivation sessions for CSMC Jesús Redentor

employees.

 Carry out extra-protocol integration activities with the entire interdisciplinary

team and collaborators of CSMC Jesús Redentor.

o Attendance at Grow.

o Training in Community Mental Health.

o Activities around the day of the elderly.

o Family Day activities.

o Activities around Spring and Youth Day.

o Activities around the international mental health day.

o Activities around the Day of the Person with Disabilities.
