STATEMENT OF WORK

USPS FITNESS CENTER MANAGEMENT

1. Background The location of the project is at the United States Postal Service Headquarters (USPS HQ) Building, 475 LEnfant Plaza, SW, Washington, DC 20260. The objective of this project is to provide a comprehensive health promotion program for USPS HQ employees as described in the requirements section of this document. The program components shall be afforded to all members and are demonstrated, in part, through the fitness and health assessments, individual programming, lifestyle counseling and educational programs, targeted interventions and facility upkeep. The components must be administered in a professional manner by the fitness center staff. Approximately, three hundred (300) USPS HQ employees use the fitness facility as active members with approximately eighty-five (85) member visits per day. The Fitness Center staff provides several exercise classes throughout the day as well as monthly lecture(s) on health and/or nutrition topics. By appointment, employees can obtain dietary information, physical assessments and personal training. 2. Scope of Work The Contractor shall provide all necessary labor, equipment and supplies (other than those provided herein), to operate an on-site Fitness Center at USPS Headquarters (LEnfant Plaza). 3. Requirements The Contractor shall do the following: Provide proper supervision of the exercise floor and exercise room Oversee the cleaning and maintenance of the fitness center (provided by USPS) and Report any cleaning/maintenance deficiencies to the Contracting Officer Representative (COR). Secure and maintain a preventive maintenance contract for Fitness Center equipment. Ensure that personnel working under this order have knowledge and skill in operating an AED. Assure the upkeep of the records and ensure all members records are kept in a secure location. Verify membership prior to users using the fitness center. Provide a quarterly report that shall include the number of participants in each Health Promotion Center for the quarter and an annual report with a cumulative total for the year. Monitor and maintain fitness equipment at the Fitness Center, in accordance with the Manufacturers recommendations. NOTE: Maintenance and cleaning of the locker rooms and Fitness center space will be the responsibility of the USPS. Provide consultation and layout of any new fitness center proposed for USPS employees. Track the number of participants and request and analyze evaluations of the programs. Provide a Quarterly Report that includes the number of participants, evaluations of the programs, and recommendations for change or improvement within existing resources. 4. Programs

The Contractor shall conduct: a. Onsite Health Promotion Programs These activities shall be in accordance with the American College of Sports Medicine (ACSM) guidelines and other applicable guidelines specified below. The contractor shall perform Worksite Health Promotion activities that consist of the following: A minimum of twenty (20) health promotion programs per year. The programs should focus on stress management, nutrition and weight management, back care and cardiovascular health. The programs may consist of lectures, screenings and/or workshops. The Contractor is expected to arrange for appropriate instructions for these programs. They will oversee the development of the program content, lesson plan and be responsible for all audio-visual aids and/or handouts. b. Health Promotion Courses

A schedule and an outline of any Health Promotion program will be provided by the Contractor to the COR within five working days prior to the scheduled course. The Quarterly Report shall include the number of participants in each course provided during the quarter and an annual report with a cumulative total of participants for each course for the year, and an analysis of the participants evaluation forms by individual courses, the success rates, special problems, that may be used to improve the courses provided. Provide and subject a schedule and an outline of any training to be provided to the COR within five business days prior to the training commencing.

Provide a Quarterly Report with the following information: the number of training classes delivered, success rates, special problems encountered, which may be used to improve or change training program components. Occasionally participate in programs that are identified by the COR (approximately two per year). c. Prepare promotional materials for health education programs. Additional Courses Exercise Classes: Exercise Classes shall be supervised and shall be offered at various times. The USPS reserves the right to add or delete or change the times for aerobic classes at any time during the life of this contract. The Contractor may be asked to provide specialty non-aerobic exercise classes, (i.e. Yoga, Tai Chi, etc.), (approximately 3-4 classes offered every week it is expected that the number of exercise classes offered shall increase in accordance with the increase in usage), and stress management (approximately one course per quarter). Course attendance requirements will be proposed and agreed upon prior to offering. At least one class per quarter shall address stress management. (Historically there were approximately 208 general exercise classes conducted over the past year, to include, but are

not limited to high impact aerobics, low impact aerobics, and stretching exercise classes). d. Postal Stampede The Contractor is responsible for organizing all aspects of the Postal Stampede, which is a 5K team walk that occurs in the spring to promote physical activity among USPS HQ employees. The event generally draws three hundred (300) participants. Organization will include, but is not limited to: marketing, recruitment of sponsors, operation of the actual event, and event set up and clean up. 5. Reporting The Contractor shall:

1. Provide quarterly, semi-annual and annual reports to the COR in the established format.
The Quarterly Report shall include the following information: Brief narrative of significant events; Worksite health promotion courses (number of participants, analysis of participants evaluation forms, success rates, special problems encountered, etc.); Training classes (number, success rates, special problems encountered, etc) which may be used to improve or change training program components; Special Health Promotion Programs (number, success rates, special problems encountered, etc). Major accomplishments; Review and evaluation of programs; problems encountered with recommendations for resolution. Recommendations for improvement.

2. Document and report all accidents or injuries in the fitness center to the COR no later than one (1) business day after the incident. 3. Equipment Recommendations and Documentation ongoing and documented quarterly Health Promotion/Screening Program Plan and Schedule The Contractor shall submit a plan for the development of the health promotion/health awareness program and a schedule for its implementation within fifteen (15) working days after contract award and within thirty (30) working days prior to the exercise of an option. 4. Member Satisfaction Surveys Each December and June, a survey will be distributed to all members. The results of these surveys shall be provided to the COR for review thirty (30) days after compilation.

6. Maintenance and Repair of Fitness Equipment The Contractor shall: Provide on-site preventive maintenance and repair services at the USPS Fitness Center. Contractor shall inspect gym equipment(s) on a monthly basis and perform preventive maintenance sessions, as required. In addition, the contractor has up to forty-eight (48) hours to respond to emergency situations (maintenance and repair if the fitness equipment). Contractor shall accomplish preventive maintenance on all cardiovascular and weight training equipment on the day of the scheduled session. The contractor exercises control over his/her work schedule on the day of the scheduled maintenance. All repairs will be done on site, during the hours of operation Monday through Friday. Replace, repair, or order additional equipment to supplement the equipment provided as USPS-furnished after obtaining written approval of the Contracting Officer (CO). Contractor will arrange for routine and necessary maintenance and repair of all equipment to ensure that it is operating consistent with manufacturers specifications and in a safe working condition. Contractor shall immediately remove from operation and post notice on any equipment that has been deemed unsafe due to malfunction until the subject equipment is either replaced or repaired. Contractor shall obtain advance approval from the COR on repair or replacement of any equipment that is deemed unsafe. The request shall include information pertaining to the condition, age, and use of the equipment, so the COR can determine if it is in the best interest of USPS to repair or replace the fitness equipment. Contractor shall perform daily inspections of the fitness center space, offices, and locker rooms for cleanliness, conditions of finishes and janitorial services. Contractor shall report any discrepancies to the COR. Contractor shall maintain records of each machine to include all maintenance and repairs required. All entries should be dated and man-hours recorded. The information will provide a history for each machine to be used in considerations for budgeting, reordering and replacement of machinery. Records will remain the property of, and are subject to exclusive control of the USPS. Contractor shall comply with all manufacturer(s) equipment maintenance guidelines and specifications to include but not limited to, wiping down all of the fitness equipment, lubricating of all bars and chains, sweeping and mopping under all machines, cleaning wax nozzles, inspecting all machines, and tightening all nuts and bolts. The Contractor shall not make any repairs or maintain equipment in such a way that will violate any expressed or implied warranties.

7. Personnel The USPS requires on-site contractor personnel to manage the fitness center from 6:00 a.m. until 7:00 p.m. Monday through Friday, excluding Federal holidays. During peak hours 11:00 a.m. 1:30 p.m. and 4:00 p.m. 6:30 p.m., staffing shall include at least one exercise equipment instructor. If the scheduled Contractor is unable to attend to his/her shift due to illness, vacation, leave of absence or turnover, replacement staff shall be provided. Support staff must have certification from a recognized industry organization (e.g., ACSM) All certifications are at the contractor or individual instructors expense. All staff shall be CPR certified with AED competency. Any uncertified employees must be supervised by a certified individual at all times. 8. Key Personnel The fitness center managers are the key personnel and the proposed individuals must have the following qualifications: o o CPR/First Aid Certification/AED Minimum of a bachelors degree in a health/fitness-related field Certification from recognized industry organization (e.g., ACSM) All certifications are at the contractor or individual instructors expense.

The Contractor shall provide a copy of each instructors certification(s) to the maintained on file in the fitness center. All certifications must be on file prior to instructing any classes. 9. 10. USPS Furnished Facilities and USPS Furnished Property The USPS will provide the fitness center for contract employees to fulfill the requirements of the Statement of Work. Please see Attachment A for a list of USPS Furnished Property. 11. Travel USPS will not reimburse for daily work commute for any contractor employees.

There will be no required travel unless pre-approved by the COR and all travel shall be in accordance with Handbook F-15, Travel and Relocation and shall be approved by the COR and CO before travel begins. 12. Leaner, Greener, Faster Smarter USPS Sustainability Initiative is USPS response to reducing its footprint on the environment. The three pillars of Sustainability are: 1) our people; 2) our network; and 3) our commitment. Worksite health promotion programs will be aligned with the sustainability three pillars. The Contractor shall be responsive to new and innovative approaches in addressing activities to fulfill the USPS Sustainability Initiative. 13. Other Meetings and Committees The Contractor may be requested to attend advisory meetings and work with other groups as designated by the COR. The Contractor shall provide publicity for all fitness center activities and programs. Publicity shall include electronic mail, posters, flyers, handouts, briefings at meetings, and other appropriate actions. The publicity shall be positive, creative and aggressive in order to attract the largest number of employees. Use of USPS media and facilities shall be coordinated with the COR. The USPS reserves the right for editorial review and approval prior to use or distribution. The USPS reserves the right to add or delete agreed upon services as appropriate at any time during the life of the contract. Such additions and deletions will be accomplished by modification to the contract.
