Hereunder is the template on Grading System for Semestral Program consistent with the Memorandum
issued by the Academic Affairs Department (ref 2005-0411-AAD-096) copy hereto attached, and will be
implemented effective this Academic Year 2005-2006.

It may be noted that the template is compressed for purposes of appreciation, and comes both on how to
arrive a grade for Lecture as well as that of Lecture and Laboratory.

1) Take note that there are templates: (1) for Lecture and (2) for Lecture with Lab

2) Ideally, the Faculty members should be provided with this soft copy to facilitate the entries, while
they submit a hard copy in triplicate, except if the Branch would have a Dean, quadruplicate is a
must with complete attachments.

3) Any erasures, if and when, must be countersigned at once.

4) Fill up all the entries needed.

5) All required signatories should affix their signature in ink (not pencil)

6) While the template is automatic, the final grade should not be rounded of to the nearest tens,and
what is left is for the Faculty members to manually encode the correct transmutation at the intended
column.

Hence, 74.98 = 74.00 = 5.00 = Failed or 91.98 = 91.00 = 1.75 = Passed

7) On NFE remarks, follow the existing policy.
