Payment InIormation The above named agrees to pay the
perIormer the agreed Iee oI $500.00 as payment Ior
services rendered.

AN ADVANCE PAYMENT OF FEE QUOTED IS
REQUIRED. UPON SIGNING OF THIS AGREEMENT
AND RECEIPT OF PAYMENT, THIS ENGAGEMENT
SHALL BE CONSIDERED BINDING. SHOULD
CANCELLATION BY THE CLIENT BE NECESSARY,
APPERANCE FEE SHALL BE FORFEITED TO THE DJ.
SHOULD CANCELLATION BY THE DJ BE
NECESSARY, THE DJ SHALL MAKE EVERY EFFORT
TO ARRANGE FOR A COMPARABLE SUBSTITUTE.
IF NO SUBSTITUTE IS DESIRED BY CLIENT, DJ
SHALL REFUND DEPOSIT IN FULL.

OVERTIME: II and when agreeable to the client and the
contracted entertainment, the time oI engagement may be
extended at the pro-ratable overtime rate oI 400.00 per
hour, to be paid prior to the overtime period.
CANCELLATION oI the engagement by the client Ior any
reason shall IorIeit the Iee paid.

Payment oI the Iee will not be required iI the act obtains a
suitable substitute booking elsewhere, or iI cancellation is
due to a veriIiable death in the immediate Iamily oI the
client.
Cancellation by the client due to a major snow storm that is
in progress on the date oI engagement must be received no
later than
Noon on the date oI the engagement.
A client cancelling their engagement by telephone must
also veriIy that cancellation in writing (via dated mail or
FAX).
Permits The client is responsible to obtain all necessary
permits Irom their town, county or state. Full payment is
due even iI an event is cancelled, delayed or postponed
because oI improper permits.

Venue and set-up. The client is responsible Ior the
provision oI a suitable venue meeting all city, state and
Iederal ordinances required Ior the event.
All equipment, electrical and musical needed shall be
provided by the client.
