C. CANCELLATION

Please note: if you do not answer the above questions it will be taken by the Institute that you are certifying that there were no reasons (eg, fit and proper/criminal/disciplinary matters) at the training contracts termination why the student, upon satisfactory completion of approved training, should not be admitted to Institute membership.
