For the past 3 months and as of now, the clients digestive functioning is still good (no loss of appetite) as she verbalized Haan nak met marigatan nga mangan, mayat pelat pannangan ko. She failed to recall her weight 3 months ago, however, last July; her weight was 60 kg, respectively. The clients mobility is good; she goes out whenever she wants to. During the past 3 months, she didnt suffer from any psychological stress or acute disease. However, she mentioned she had headache and elevated blood pressure a year ago and managed it by taking Lozartan 50 mg OD and Mefenamic Acid. With regards to her neuropsy chronological problems, she considered and admitted that sometimes, she experiences mild dementia or moderate memory loss as she verbalized Haan met maiwasan ta bumaket met da taon ngem haan met unay nakaro. No pressure ulcers present. According to her, she eats full meal thrice a day. She drinks 1 glass of milk a day, eats 2 or more eggs a week and seldom eats fish, meat or poultry, at least once a month only for she prefers eating vegetables. As claimed by the client, she drinks about 1.5 liters of fluids including water, juice and milk. She can prepare her meals, feeds self alone and views self with no nutritional problem.

Analysis: For the screening and assessment, the client garnered 23 points, having no risk for any nutritional problems since 16-22.5 points is the range of having risk for malnutrition and points lesser than 16 is malnourished. Since the client is growing older, at the age of 71, she would experience mild dementia due to the decreasing size of the brain neurons. III. Psychosocial Mrs. Carlina Corpuz, 71 years old, elementary graduate, was a farmer for more than 40 years. She claimed that her occupation was exhausting and felt relief when she stopped working when she was 30 years old. As of now, she accepts sewing services for her living. The client participates in different social and leisure activities such as joining street dancing and field trips. She involved herself in different organizations such as Senior Citizens, Widows and Womens club, as enumerated by the client. With her engagement with these organizations accompanied by different activities, absence of diseases and stable socioeconomic status, she claimed that her life as of today is somewhat satisfying, with moderate self-esteem.
