0 STUDENT-ATHLETE ACADEMIC SUCCESS AGREEMENT

The Lincoln High School Football Program is committed to your academic success as a student-athlete. We firmly believe that education is the path to success and, as a result, we treat academic achievement as a prerequisite to athletic participation. Our MISSION 3.0 goal is for every student-athlete to complete the season with a 3.0 GPA or higher. As a team, we can and we will accomplish MISSION 3.0 together!! MISSION 3.0 holds each student-athlete towards a higher-than-average academic standards. The following Agreement applies to all football players and describes what will be expected in order to participate in football activities. PLEASE NOTE: If you fail to maintain this agreement, you will be impacting your teammates, your school, and your community because you will be missing large parts of practice, as well as game-time! This will directly affect our success on the football field. The only way we can be a successful FOOTBALL program is if all of our players are successful STUDENTS!

MISSION 3.0 STUDENT-ATHLETE ACADEMIC SUCCESS AGREEMENT

I, __________________________, agree to the following goals & requirements of Mission 3.0 as a part Student-Athletes Name of my to participate in the Lincoln High School Football Program.

1.

CLASSROOM EXPECTATION: As a student-athlete, I understand that how I perform in the classroom will determine my level of participation in practice and even in games. I will sit in the front row of every class, and I will behave with RESPECT and HARD WORK for each one of my teachers. I will not be a distraction in the classroom, since I am aware that my behavior in school reflects on my team, my coaches and my family.

STUDY HALL: As a student-athlete, I will participate in the MISSION 3.0 Study Hall after practice on Mondays, Tuesdays & Thursdays. During Study Hall I will come prepared with either work from class to finish or study material to prepare for my exams. As a student-athlete, I agree to work for the success of my teammates, and to work under the supervision of my coaches. I will sit where assigned and will not distract my teammates. I fully understand that my attendance in the MISSION 3.0 Study Hall = being eligible to play in that weeks game. 3. ASSIGNMENT LOG: In order to achieve MISSION 3.0 success, I will maintain an assignment log for every class and agree to complete every assignment for all my classes. I will BE RESPONSIBLE to confirm this each week by having an assignment log COMPLETED and then SIGNED by each of my teachers, as well as by my parent(s)/guardian(s). I fully understand that COMPLETED assignment log = being eligible to play in that weeks game. 4. PROGRESS MONITORING: I will turn in an academic progress report for MISSION 3.0 to my coach. I fully understand that my completed progress report = being eligible to play in that weeks game. 5. ACADEMIC SPONSOR: I will ask one of my teachers past or present to be my MISSION 3.0 sponsor. This teacher will wear my jersey on game day during Homecoming week, and I will be in regular contact with them to get help in achieving Mission 3.0 success. I wholeheartedly agree to these expectations and I understand that if I perform below a 3.0 or do not maintaining an assignment log or complete my assignments, that I will put academics ahead of athletics to complete my assignments and will not resume practice or play in any games until I have done so.
