Ref:- Approval of Vice-Chancellor dt. 26-09-2011 @@@ The Univeresity has recognised the following 39 hospitals as referral hospitals for the purpose of inpatient treatment and outpatient treatment. These hospitals will charge CGHS tariff for in-patient treatment.

The terms and conditions of Memoradum of Agreements as entered into with the above hospitals are as follows. a) The Hospital shall charge the tariff as prescribed and revised by the CGHS from time to time. b) If the tariff for any given procedure is lower than the CGHS tariff, the hospital shall charge the actual tariff. c) If the procedure is not covered by CGHS then the hospital shall charge the tariff as prescribed by the NIMS, Hyderabad. d) In case the procedure is not covered by the NIMS tariff rates also, then the hospital will charge its actual tariff. e) The conditions stated at (c) and (d) above are not applicable to Apollo Hospital. The Apollo Hospital will offer a standard 20% discount on its actual tariff for the procedures not covered by CGHS tariff. Instructions to University Employees: a) You may approach any approved/notified registered medical practitioner for outpatient treatment. But the consultation charges and the resultant outpatient treatment charges will be restricted to CGHS tariff for reimbursement. The retired employees however may draw an allowance of Rs. 300/- p.m. for outpatient treatment or avail the service of University Health Centre. The option exercised in this regard will be final and binding. They cannot avail both the facilities. b) When you go to the hospital for admission of yourself or any member of your family as an in-patient, inform the hospital authorities of your being an University employee and of your pay so as to enable the authorities of the hospital to allot you the accommodation as per your status and eligibility. c) The procedure hitherto followed with regard to drawing of Medical Advance; Adjustments of Medical Advance and submission of Medical Bills Reimbursement claims remain the same. d) At the time of leaving the hospital after treatment, please get the hospital bill and receipts, vouchers, Essentiality Certificate, etc., duly signed or countersigned by the Medical Officer in- charge of the patient in the hospital, as the case may be, for the purpose of claiming reimbursement of expenses incurred and refund from the hospital if advance is paid. e) In emergent cases involving accidents, serious nature of diseases, etc., the patient can be admitted in a private hospital/clinic in case no hospital recognized by the University is available closer than the private hospital/clinic and the case is one of real emergency necessitating such admission and treatment. f) Prefer your claims for reimbursement of medical expenses incurred as per CCS(MA) rules, in the relevant application form, giving full particulars called for therein and also attaching

all the certificates required to be produced under the rules. This will avoid delays in settling your claims. g) All inpatient bills should be submitted within a month from the date of discharge from the hospital. h) All inpatient claim bills should contain the original discharge summary in addition to other certificates as required.

All bills are processed as per CGHS rates/CCS (MA) rules and no amount will be reimbursed towards inadmissible expenses/excess amount charged towards higher category of their entitlement. All employees must aware of their entitlement to avoid recovery.

j) For any reasons whatsoever it may be, if hospital authorities charge more than CGHS rates, bills will be processed only as per CGHS rates and any excess amounts charged by the hospital authorities are to be borne by the employees. However in such cases Health Centre will take lead to resolve such issues and to get the refund from the hospital authorites to the extent possible. k) If employee opts for higher class of accommodation, bills will be reimbursed only as per their entitlement. l) All retired employees are not entitled for Fixed Medical Allowance of Rs. 300/-. Only pensioners are eligible for payment of Rs. 300/- OR alternatively then can opt for Health Centre services.

Sub:- Recognition of Hospitals as referral hospitals Reg. Ref:- 1.Circular No. UH/Health Centre/Hosp.Recog/2011/1328, dt. 27 October, 2011. 2. Vice-Chancellors Orders dt.20-10-2011 @@@ In continuation of the Circular 1st cited on the subject, the following 10 hospitals have also been recognized as referral hospitals for the purpose inpatient treatmnent and outpatient treatment. These hospitals will charge CGHS tariff for the in-patient treatment. 1. F.M.S. Dental Hospital (Koti, Punjagutta, Kukatpally, Dr. A.S. Rao Nagar, Dilsukhnagar, Lungar House and Secunderabad branches). The Hospital will charge CGHS tariff for all the procedures except for the following four procedures for which it will charge the tariff as approved by the Govt. of Andhra Pradesh The differential cost arises out of undergoing treatment for these procedures (between CGHS tariff and the actual tariff charged by the hospital) will have to be borne by the patient only. a) Flap Surgery; Restoration. b) Root Canl Treatment; c) Metal Crown; and d) Composite

The following hospitals / diagnostic centres did not agree to extend the facilities to the beneficiaries of the University at the CGHS tariff rates. However, keeping in view of their area of specialization of treatments/diagnostics and availability of sufficient infrastructural facilities, it has been decided by the University that the beneficiaries of the University may be given freedom to go to these hospitals/diagnostic centres at their own risk and decision but the reimbursement shall be restricted strictly to CGHS tariff only and no medical advance should be given for treatment in these hospitals. a) Swapna Hospital, Raj Bhavan Road; b) Fernadez Hospital, Boggulkunta;

c) L.V. Prasad Eye Institute, Banjara Hills; d) Mohans Diabetic Centre, Ramakrishna Mutt Road (it has been informed by the Hospital that it will offer 20% discount for out-patient treatment and 10% discount on in-patient treatment); and e) Focus Diagnostic Centre, Punjagutta (it has been informed by the Diagnostic Centre that it will offer 20% discount on all investigative procedures).
