aureus, Streptococcus pyogenes, Proteus mirabilis Uncomplicated UTIs Prostatitis due to E. coli Contraindications Contraindicated with allergy to fluoroquinolones, lactation Use cautiously with renal impairment, seizures, pregnancy CNS: Headache, dizziness, insomnia, fatigue, somnolence, depression, blurred vision Adverse Effects GI: Nausea, vomiting, dry mouth, diarrhea, abdominal pain Hematologic: Elevated BUN, AST, ALT, serum creatinine and alkaline phosphatase; decreased WBC count, neutrophil count, Hct Other: Fever, rash, photosensitivity Nursing Responsibilities Assess clients allergy to fluoroquinones, renal impairment, seizures, lactation, pregnancy Assess patients skin color, lesions; T; orientation, reflexes; mucous membranes, bowel sounds; LFTs, renal function tests Arrange for culture and sensitivity tests before beginning therapy Continue therapy for 2 days after the signs of infection have disappeared Administer oral drug 1 hr before or 2 hr after meals with a glass of water

Ensure that patient is well hydrated Administer antacids at least 2 hr before or after dosing Monitor clinical response; if no improvement is seen or a relapse occurs, repeat culture and sensitivity tests Instruct client to take drug on an empty stomach, 1 hr before or 2 hrs after meals. If an antacid is needed, do not take it within 2 hrs of ofloxacin dose Use eye or ear drops as instructed; keep tip away from surfaces; wash hands before administering Drink plenty of foods Avoid prolonged exposure to sunlight Inform patient that he may experience side effects like nausea, vomiting, abdominal pain (eat frequent small meals); diarrhea or constipation; drowsiness blurring of vision, dizziness Instruct to report rash, visual changes, severe GI problems, weakness, tremors
