JOINT AFFIDAVIT OF ARREST

WE, POI ARIEL VILLEGAS , POI ERIC JACALAN , POI HALDIGARD ROMA and POI RAMON BUTIL , bona fide members of the Philippine National Police presently assigned with Lapu-lapu City Police Office, Lapu-lapu City, Philippines, after being sworn to in the manner prescribed by law, do hereby depose and say; That we are detailed as Team Leader and Members, respectively, of the Alert Team of Police Station 4 situated in Marigondon, Lapu-lapu City; That on April 18, 2010 at around 2:20 oclock in the afternoon we received a call coming from a certain Cherry Faith Donque informing us about a person went into her house which is located at Simplex , Suba Basbas , Lapu Lapu City and created trouble / violence thereat by banging many times of her entrance gate ; That instance , we proceeded to the described place and upon our arrival we saw a person who was presumably drunk who already inside , always banging the gate and shouting to the top of his voice against somebody inside the house ; That we lawfully ordered the suspect to stop his unlawful act that made the people around fear , however , he never heed our lawful orders , instead , he threatened and challenged us about our profession , but we patiently cautioned the suspect to calm down but he ignored and defied ; which resulted to a brief scuffle and finally he was subdued ; That having reasonable grounds to believe that he committed a crime in our presence, we put him under arrest , informed the nature of his offense committed as well as his basic rights under the law --- and subsequently brought him to Lapu-lapu City Hospital for liquor test examination which gave positive result , then to our main office for proper disposition; That at the office arrested suspect identified himself as FERNANDO DE LEON , 46 years old , married and a resident of Marbeach , Marigondon , Lapu Lapu City ; That we are executing this affidavit to support in filing of appropriate charges against FERNANDO DE LEON in the Court of Justice; IN WITNESS WHEREOF, I have hereunto set our hands this 19th day of April 2010 at the City of Lapu-lapu, Philippines. JOINT AFFIDAVIT OF ARREST

WE, POI MELVIN S. PATOC and ALMER CATAPUSAN , all neophyte members of the Philippine National Police presently assigned with Lapu-lapu City Police Office, Lapu-lapu City, Philippines, after being sworn to in the manner prescribed by law, do hereby depose and say; That we are detailed as Beat Patrol at Tamiya , Basak , Lapu-lapu City; That on July 4, 2010 at around 12:20 oclock midnight while performing our official duties at Tamiya , Basak , Lapu Lapu City particularly fronting BFK Karaoke Bar , our attention were caught when we noticed a man and a woman having a verbal tussle with each other and this man himself is seemingly under the influence of intoxicating liquor ; That moment , we keep on eyeing and observing their movements because we speculated that there might be a misunderstanding between the duo. And in a few minutes later , in an angry manner the man hold and choked the neck of the woman that made the former uneasy to breath; That this time , we lawfully ordered the man to stop about his unlawful act , however , he never heed our lawful orders , instead , he threatened and challenged about our profession , but we remain calm and patiently advised the suspect to calm down but he ignored it --- and with the used of our force , a brief scuffle was ensued and finally he was subdued. During scuffling my ( POI Catapusan ) personal holster which was tucked as part of my uniform worth One Thousand Eight (P1,800.00) Pesos was broken when the suspect resisted and assaulted us; That having reasonable grounds to believe that he (suspect) committed a crime in our presence, we put him under arrest , informed the nature of the offense he committed as well as his basic rights under the law --- and subsequently brought him to Lapu-lapu City Hospital for a liquor test examination which gave positive result , then turned him over to our main office for proper disposition; That at the office arrested suspect identified himself as DENNIS PALANG SO , 39 years old , married and a resident of Sudlon , Maguikay , Mandaue. While in the process of interviewing between parties , arrested suspect still went unruly by violently yelling to the top of his voice demanded the police to set him free , but we and the duty policemen who witnessed politely advised the suspect to calm him down but he insisted / defied and further he kicked the Investigators table and some of the properties around , thus causing damaged of undetermined amount; That we are executing this affidavit to support in filing of appropriate charges against DENNIS PALANG SO for the crime/ violation of Direct Assault , Alarm and Scandal and City Ordinance ( Drunkenness ) in the Court of Justice; IN WITNESS WHEREOF, I have hereunto set our hands this 5th day of July 2010 at the City of Lapu-lapu, Philippines.

Quezon St., Cabancalan, Mandaue City, Philippines, after being sworn to in the manner prescribed by law, do hereby depose and say; That I am a taxi driver of Spider Taxi bearing plate number GWX 357; That on March 3, 2009 at around 1:00 oclock in the morning, a Korean passenger embarked the taxicab at Maribago, Lapu-lapu City for Boyla Hotel, Buyong, Maribago, Lapu-lapu City and upon reaching the said place, said passenger whom later identified as LEE YOUNG SU disembarked and did not pay the taxi fare amounting to PHP 40.00calan, Mandaue City personally appeared at our station together with a Korean passenger identified as Lee Young Su, 35 yeas old, single, temporarily residing at Humay-humay St., Gun-ob, Lapu-lapu City; That above-named driver alleged that said suspect embarked the taxicab for more than three hours and did not pay his taxi fare reaching the amount of PHP 300.00, moreover, suspect (Lee Young Su) vomiting inside the taxicab and stopped somewhere in Bagy Maribago disembarked and kicked the back portion of the taxicab which caused damaged of un-estimated amount; That we lawfully ordered Lee Young Su to disembarked inside the taxicab, however, the latter disobeyed and defied our lawful order which resulted a brief scuffled but we were able to subdue the suspect after the said scuffling and placed inside the patrol car; That having reasonable ground to believe that said Korean has committed a crime in or presence, we put him under arrest, informed the nature of his offense committed as well as his basic rights under the law and subsequently brought him to Lapu-lapu City Hospital for liquor test examination which gave positive result then to our main office for proper disposition; That all of the foregoing facts and circumstances are true and correct to the best of our knowledge and belief; That we are executing this affidavit to support in filing of appropriate charges against LEE YOUNG SU in the Court of Justice; IN WITNESS WHEREOF, I have hereunto set our hands this 3rd day of March 2009 at the City of Lapu-lapu, Philippines. AFFIDAVIT OF ARREST/COMPLAINT
I, TOMAS GOMEZ MATUGAS, 29 years old, married, a taxi driver and a resident with postal address at Ponce Capitol Hills, Cebu City, Philippines, after being sworn to in the manner prescribed by law, do hereby depose and say; That I am a taxi driver of Scorpion Taxi bearing plate number GWX 921; That on May 3, 2009, at around 11:40 oclock in the morning a passenger flagged me down and convey from Barangay Pasil Cebu City bounding to Lapu-lapu City and upon reaching to the said destination, the said passenger intentionally disembarked from the taxi and without paying his taxi fare while at the crossing of Barangay Pajo, Lapulapu City amounting to PHP 397.00 then ran away; That I immediately ran after him and chase the passenger whom trying to evade from me but luckily I was able to subdue the said passenger and arrest him (passenger) and I subsequently turn over him at Lapu-lapu City Police Office for proper disposition; At this juncture, an investigation was conducted at General Investigation Desk by P01 Ramon Velez IV as duty investigator and found out that the said passenger defrauded me to prejudice and damage with the grave abuse of confidence on my part thereof the total amount of PHP 397.00 and when we let him pay the taxi fare, instead he failed to give any amount; As a consequence for not paying his taxi fare, it resulted to his arrest and the said passenger whom later identified himself as one ARNOLD ARAAS PATALINGHUG 34 years old, single, a Hair Dresser and a resident of Seaweeds, Barangay Pusok, Lapulapu City; That all of the foregoing facts and circumstances are true and correct to the best of my knowledge and belief; That I am executing this affidavit to support in filing appropriate charges against the above named suspect for the crime of ESTAFA in the Court of Justice; IN WITNESS WHEREOF, I have hereunto set my hands this 4th day of May 2009 at the City of Lapu-lapu, Philippines.
