cues y Subjective: makulog ang hita ko pagnahihiro as verbalized by the patient. Nsg. Diagnosis y Pain r/t breach of continuity of the bone y Inference Pain is one of the major manifestation of fracture y Objectives Manage pain y Nsg. Intervention Interdependent: administration of acetaminophen y Rationale Acetaminophen relieves pain by blocking pain impulses and inhibits production of prostaglandin evaluation y After admin. Of drugs pain is managed as manifested by good mood of the patient After ROM activities are done there is a marked increase in mobility of body parts not affected by the injury After cast or mold is applied the safety of the patient is improved

Activity intolerance r/t disease process

Fracture cause immobility and loss of function of the affected area

Promote mobility of body parts not affected by the injury

Independent: Perform ROM

ROM prevents muscle atrophy that may be acquired because of immobility

Objective: xray result shows complete close fracture

Risk for further injury r/t breach of continuity of the bone

Fractured bone can further make an injury if not immobilized

To decrease the risk for injury

Interdependent: immobilize the injured site by applying a cast or a mold

Immobilization of the injured area can decrease the risk of injury by preventing the bone from puncturing the skin causing more damage
