Results: From endoscopic ndings, the relationship of nerve and vessel was as follows: no compression was in 4.49% (7/156) patients; just contact was in 32.05% (50/156) patients; exact compression was in 63.46% (99/156) patients. Out of 147 cases in which trigeminal nerve was related with vessels, the offending vessels were SCA (51.01%), AICA (20.13%), VA (6.04%), PV (2.68%), multiple vessels (15.44%) and unknown vessels (4.70%). The types of compression were: simple conict (70.47%), multiple conict (18.12%), sandwich conict (5.37%) and paralleled conict (6.04%). The sites of conict were REZ (42.95%), cisternal portion (40.27%), entry to Meckels cave (4.03%), paralleled (6.04%) and more than two sites (6.71%). Conclusion: Endoscopes used in microvascular decompression can increase the clarity of the surgical eld and simultaneously get panoramic visualization without black corner to avoid omitting offending vessels. In addition to pinpointing the location of NVC, endoscopes can also ensure the exact relation of the nerve and vessels. doi:10.1016/j.ijom.2007.08.223

Obstructive sleep apnoea syndrome: prospective study of 15 patients treated by maxillomandibular advancement with counterclockwise rotation osteotomy J. F. Cifuentes School of Dentistry, Faculty of Medicine, Universidad del Desarrollo-Clanica Alemana, Santiago, Chile It is well documented that maxillomandibular advancement (MMA) with counter clockwise rotation (CCW) of mandible and maxilla can improve airway dynamics and allow for more efcient air-ow in patients with obstructive sleep apnoea syndrome (OSA). Purpose: This prospective study was conducted using clinical, radiological and polisomnographic evidence to compare parameters before and after surgery, in patients treated with MMA with CCW rotation. Method: Fifteen patients with moderate to severe OSA and Class II maloclussion were evaluated before and after MMA with CCW rotation doing clinical evaluation, lateral cephalometric radiology and basal polisomnography (Respiratory Disturbance Index, RDI; Lowest Oxygen Desaturation, LSAT and Body Mass Index, BMI). Result: Out of the 15 patients studied, 14 had important improvements in the mentioned parameters and were successful as dened by the Stanford group. And one patients condition was partially improved. In this series, success rate is 93%. Conclusion: MMA with CCW rotation achieved expansion of the upper airway. CCW rotation osteotomy allows better facial harmony results than when MMA advancement is performed alone for the treatment of these patients. Forty subjects, in which 20 subjects diagnosed as OSAS polysomnographically as experimental group, and 20 non-snoring agesexmatched normal subjects as a control group were used in this study. The cross-sectional area of retropalatal region (RP), retroglossal region (RG) and epiglottal region (EPG) was calculated. The thickness of lateral and posterior pharyngeal wall and other indices were also calculated. In OSAS group, the AP/LR ratio and the thickness of lateral pharyngeal wall in RP, RG, EPG regions, the length, thickness and cross-sectional area of the soft palate were statistically larger than those of the control group (P < 0.01). The study suggests that the pathogenesis of OSAS is related to the following factors: AP/LR ratio, cross-sectional area of airway, the thickness of lateral and posterior pharyngeal wall and other indices. doi:10.1016/j.ijom.2007.08.226

Centbucridine a novel local anaesthetic versus lignocaine for minor oral surgical procedures M. T. Muthu*, M. Veerabahu, B. Vikraman Dept. CH-600042, India Objective: Search for more effective, safer, low-dose l.a had lead to development of centbucridine which is mainly developed in India by Indians. It has inherent vasoconstrictor effect and does not need adrenaline and hence minus its side effects. Its efcacy and safety has been proved with its use in subdural anaes and ocular anaes but it is not widely used for dental anaesthesia for no reason. We used it and found good results. Methods: We used double blind clinical study to evaluate onset, duration, depth, safety of 0.5% centbucridine compared with 2% lignocaine. A random group of 30 patients was selected for this study. Results: Centbucridine had equivalent efcacy, onset and duration and had lesser CNS and CVS side effects compared to lignocaine. Analgesic effects of topical 0.075% capsaicin with oral carbamezipine in idiopathic trigeminal neuralgia M. Ali*, S. M. It usually occurs in the distributions of trigeminal nerve. The incidence is reported to be 35 per 100,000. Capsaicin has a veried analgesic effect in post herpetic neuralgia, diabetic neuropathy, and surgical neuropathy. Objectives: To compare the analgesic effects of topical 0.075% capsaicin with oral carbamezipine in idiopathic trigeminal neuralgia. Result: Pain score was signicantly reduced innon exposed group at rst, second, third and fourth week while in exposed group the overall pain was reduced in rst week but gradually decreased in second, third and fourth week as the dose of carbamezipine reduced. Conclusion: Carbamezipine is better than capsaicin in pain controlling effects in the management of trigeminal neuralgia. doi:10.1016/j.ijom.2007.08.224
