The following questionnaire is part of my leadership course at the university. The questions are written in a way to inquire if I possess specific qualities or values that successful leaders should possess, or if I work according to certain procedures that successful leaders should follow.

The answer code consists of 5 terms ranging from disagreeing to agreeing with the given statement. Answer code: 1 - Disagree 2 - Partially disagree 3 - Neutral 4 - Partially agree 5 Agree Note: If there are any additional statements that you would like to make to clarify something, you can write it in the comments section.
I have used four levels of leadership which I have borrowed from the book Principle Centered Leadership by Covey: 1- Personal level : my relationship with myself 2- Interpersonal level: my relationship & interaction with others 3- Managerial level: my responsibility to get a job done with others 4- Organizational level: my need to organize people & coordinate work for collective purposes
I have a clear vision for my life in general which is apparent in the decisions that I have made so far, and I know where Im heading and what I want to accomplish in every task I do in particular.
I have immense amount of energy level in me, which I show off in every task that is handed to me. I have the drive and enthusiasm to perform even the most difficult tasks in the most difficult situations.
I possess integrity, that is I keep promises that I make either to myself or to others, in other words, I take responsibility for everything I say and promise, and I work accordingly, not doing whats the opposite.
My actions demonstrate the level of maturity that I have reached, through courageous decisions that I have made in tough situations, or when I have shown kind & compassionate behavior towards others.
I work having abundance mentality, that is I believe life is not competition, where there is only one winner, but there is place for everyone to succeed and excel. This belief I have shown through my actions, where I help others to succeed as well and take credits, not obstructing their way and competing with them to take all the credits to myself.
I always show positive attitude and optimism in all situation, even in the worst of them.
I have boundless curiosity, and I like to know about everything even though it might not be in my domain of work or interest.
I have self-confidence and I feel secure about myself and what I know and I can do.
I have broad education, and I continue and strive to learn as much as I can.
I possess conceptual knowledge, that is the ability to conceptualize issues, even in fields that are not my specialty.

The following four attributes create a leaders voice, which characterizes his unique personal significance, how would you evaluate me in these attributes:
TALENT: I have different kinds of strengths and gifts that I put into effective use while Im performing a specific action, or working in general. Response: Disagree / Partially Disagree / Neutral / Partially Agree / Agree
PASSION: While working, I show passion & interest in the work Im doing, and I develop motivation and excitement that inspire not only me but others as well. Response: Disagree / Partially Disagree / Neutral / Partially Agree / Agree
NEED: My contributions to the work done so far imply that there is the need for me in the workplace, or for example, in any situation, my presence has clearly helped things to go more smoothly and be accomplished more quickly. CONSCIENCE: Every job I perform, I do it according to my conscience, differentiating between what is right & wrong, and doing only what is in accordance with my beliefs. Response: Disagree / Partially Disagree / Neutral / Partially Agree / Agree Comments:
When in debate or discussion, I try to understand others, before expressing myself; in other words, I dont impose my personal opinion on others right away.
I try my best to preserve my relationship with others, not offending anyone despite of differences. I try to change negative arguments into more constructive ones.
I show empathy and understanding with respect to people and whatever type of situation they can be in, I try to be compassionate and understand their conditions.
I possess teaching abilities, for example, I have been able to teach people or pass on knowledge, so that they can also use it and apply it in their jobs.

I plan my goals systematically plan & develop appropriate strategies to achieve my goals.
I focus on the goals that I have made, I prioritize them, and I work accordingly.
I have clear judgment on different situations, I dont get biased, but I judge according to my beliefs and values.
I possess the tendency for risk-taking, and I have made decisions that required taking risk, and I have taken them without hesitation, knowing they would be best under the circumstances.

I have a level of self-empowerment, the ability to make decisions that helps me take initiative in certain situations.
When it seems there are only two choices to be made, I am able to find a third possible alternative, which reveals itself to be a more synergistic and perfect solution.
I have technical competences, skills, which allow me to perform the tasks entrusted to me smoothly.
I plan my work as to achieve long-term growth, rather than small temporary short-term achievements.
I have disciplined lifestyle, I stay focused on my goals, I organize the things I have to accomplish, and work accordingly, I dont make excuses, and try to focus on results.
In many situations, I have showed that I possess problem-solving ability, and actually taken responsibility and solved them.
I can adapt to the working environment easily, and work with new people or in new circumstances effectively and smoothly.

I communicate any vision I have for a specific task to the people who are affected by it.
During work, I make sure that the procedures are always aligned with prior set goals and principles.
I am generous with the people I get in touch with, for example I provide them with assistance that I have learned prior that might be useful to them in certain situations.
I belief in teamwork, and that people can work together. I do my best when working in teams, taking into consideration all the team members points of views without imposing myself on them.
Through my actions, I try to serve people as much as I can; I try to be helpful in situations where I can give my support or where Im needed.
I provide myself and others with motivation and empowerment. I inspire myself to get the results I want, and while working, I empower others, help them become independent decision makers, so that they feel they achieve more.
I achieve successful interdependency with others; I am able to collaborate with people, work with them and together achieve joint successful results.
