Generic Celebrex[cap]

Dose Acute pain 400mg followed by 200mg if needed on 1st day & 200mg bid or 400mg once daily on subsequent days

Side Effect Constipation; diarrhea; dizziness; gas; headache; heartburn; nausea; sore throat; stomach upset; stuffy nose. Adverse Effect Anaphylactic reactions, renal toxicity. Hallucination, ageusia, anosmia, aseptic menlugitis, vasculitis, GI hemorrhage. Hepatitis, liver failure, interstitial nephritis. photosensitivity reaction, exfoliative dermatitis, erythema multiforme, StevensJohnson syndrome, toxic epidermal necrolysis & menstrual disorder. Cerebral hemorrhage, fulminant hepatitis, liver necrosis, hyponatremia, conjunctivitis. Contraindication Contraindicated against asthma, urticaria, allergic-type reaction to sulfonamides, aspirin or other NSAIDs. Treatment of perioperative pain in the setting of coronary artery bypass graft surgery. Severe hepatic impairment.

Nsg Responsibility Before administering Celebrex, nurses must assess patients for any contraindications. Contact physician when there are bloody stools/cramping. Assessment History: hepatic and renal impairment Physical: physical status, renal function test, culture and sensitivity tests.

Indication OA , RA, Post Surgical/dental pain & acute flare pain of osteoarthritis Acute pain include primary dysmenorrhea. Familia adenomal polyposis (FAP).

Classification Nonsteroidal AntiInflammatory Drugs (NSAIDs)

Mode of action Celebrex is a nonsteroidal anti-inflammatory drug (NSAID) that exhibits anti-inflammatory, analgesic and antipyretic activities in animal models. The mechanism of action of Celebrex is believed to be due to the inhibition of prostaglandin synthesis, primarily via the inhibition of cyclooxygenase-2 (COX-2) and, at therapeutic concentrations in humans, Celebrex does not inhibit the cyclooxygenase-1 (COX-1) isoenzyme. In animal colon tumor models, celecoxib reduced the incidence and multiplicity of tumors.

Generic Prednisone

Dose Adult: PO Adjunct to treatment of Pneumocystis carinii pneumonia >13 yr and w/ AIDS: 40 mg twice daily for 5 days, followed by 40 mg once daily for 5 days, then 20 mg once daily for 11 days or until completion of the anti-infective regimen. Should be initiated w/in 24-72 hr of initial antipneumocystis therapy. Allergic conditions Initial: 30 mg on the 1st day, then taper by 5 mg/day until 21 tab have been administered. Advanced pulmonary or extrapulmonary tuberculosis 40-60 mg/day, then taper over 4-8 wk. SLE Mild: 10 mg/day. Refractory or severe: 20-60 mg/day.Rheumatoid arthritis 10 mg/day. Idiopathic thrombocytopenic purpura 1-2 mg/kg/day. Acute asthma 40-60 mg/day for 3-10 days. Side Effect Immunosuppresion Short term: high blood glucose levels, fluid retention, insomnia, euphoria and, rarely, mania long term: Cushing's syndrome, truncal weight gain, osteoporosis, glaucoma and cataracts, type II diabetes mellitus, and depression upon dose reduction or cessation.

Nsg Responsibility -Advise to take with food. -Be aware that a slight weight gain with improved appetite is expected, but after dosage is stabilized, a sudden slow but steady weight increase [2 kg (5 lb) per wk] should be reported to physician. -Avoid or minimize alcohol and caffeine may contribute to steroidulcer development in long-term therapy. Assessment History: hepatic and renal impairment Physical: physical status, renal function test, culture and sensitivity tests.

Mode of action unbound glucocorticoids readily cross cell membranes and bind with high affinity to specific cytoplasmic receptors. This binding induces a response by modifying transcription and, ultimately protein synthesis to achieve the steroid's intended action

Adverse Effect Insomnia, nervousness, increased appetite, indigestion, dizziness/lightheadedness, headache, hirsutism, hypopigmentation, diabetes mellitus, glucose intolerance, hyperglycaemia, arthralgia, cataracts, glaucoma, epistaxis, diaphoresis, Cushing's syndrome, edema, fractures, hallucinations, hypertension, muscle-wasting, osteoporosis, pancreatitis, pituitary-adrenal axis suppression, seizures. Contraindication Hypersensitivity, serious infections (except tuberculous meningitis), varicella, systemic fungal infections. Pregnant.
