Dear Vendor, I would like to invite you to the 14th World Congress on Qigong and TCM (14WCQ) to be held

from Friday May 25th, 2012 through Monday May 28th, 2012 in the hours of 9:00am to 6:00pm at Ryerson Theater (May 25th) and Hyatt Regency Toronto (May 26th to May 28th) located at 370 King Street West. This event will mark the first time the World Congress has come to Canada and we are inviting you to be a part of this historic occasion. Past congresses have been very successful with hundreds of local, out of town and out of country attendees and participants willing to spend their day in the pursuit of enhancing their spirit, mind, and body. With your support we can create a large public awareness of natural therapies and make this the biggest World Congress on Qigong and TCM to date. Vendors will have a booth space to sell, promote and demonstrate their products or services within a designated area. If you would like to participate, please fill out the application form provided and send to the address on the application. **Early entrants will have optimal vendor location choice!!! DATE & LOCATION Vendors will be permitted to setup and promote their products and/or services at the following date, time and location: Date: Saturday May 26, 2012 and Sunday May 27, 2012 Time: 9:00am to 5:00pm Location: Hyatt Regency Toronto on 370 King Street West

VENDOR PARTICIPATION & PRICING It is our intention to provide maximum value to this congress by creating an exclusive opportunity for our vendors to create awareness around their products and or services by setting up a booth to sell, promote or demonstrate your products or services. SHOW FORMAT
a) Setup will take place between 6:00am-7:00am on Sat. 26th and Sun 27th b) Tear down starts at 6:00pm Sat. 26th and Sun 27th c) Table Display: ONLY WHITE TABLE CLOTHS ALLOWED: We ask that you participate in this general theme by only using white cloth on top of your table if you need extra draping for your display. d) Banners and Signage: Banners are allowed hanging in front of your table or on any walls you bring. Please keep all your signage displayed on your table. Up to 6 foot free standing vertical banners are allowed. The vertical stands are approximately 3 feet in length with a 5 to 6 foot banner roll up. e) Vendors must receive Congress Executive Committee approval for all products or services displayed or sold. Approval will be mailed or emailed to vendor indicating approved items. Display and/or sales of unapproved items may be cause to dismiss vendor from event grounds.

f) The vendor shall maintain a clean and safe operation for the full duration of the congress. Vendor shall clean up ones booth and remove or dispose of trash. Vendors are expected to comply with all federal, state and local laws, rules and regulations, and shall have all appropriate licenses, permits and other required documentation. g) Vendor signature on the Application Form indicates acceptance of these participation rules. h) No application will be processed without full payment of registration fee. Vendor space is limited. Please submit your application as soon as possible. i) By participating in this event, the vendor agrees to indemnify and hold harmless the City of Toronto, Hyatt Toronto Regency, the World Congress on Qigong and TCM and their Executive committee, directors, members, sponsors, officers, agents, staff and employees from any and all liability for damage, City Health code violations, injury, or loss to any person or goods for any reason. The exhibitor understands that he/she is an independent party and is solely responsible for their vendor space and operation, and that the Hyatt Toronto Regency is just providing a place for the party to conduct their business. h) No additional power supplies will be allowed, unless approved by Congress Executive Committee BEFORE event date. Power supplies will be available for use for items such as freezers, toasters, etc., but require prior approval and the payment of the fee.

I acknowledge that I have read and understand the terms and conditions as indicated on this application/contract and may be subject to change. I agree to sell/promote only those items as listed in the application form that will be attached to this document. If I change/add a new category or merchandise, I must submit a request to the 14th World Congress on Qigong and TCM. Any vendor that does not comply with these rules and regulations will refuse the application form.
