ACTIVITIES OF DAILY LIVING Activities Nutrition Before Hospitalization Before, the client eats 2-3 times a day.

Elimination

Before he urinates 5-7 times a day with difficulty and the color is very yellow. Before he defecates every 3 days and his stool is hard and formed.

Now that he is hospitalized, he urinates about 2-3 times a day and the color is slightly yellow with pain and difficulty. Now he defecates 2 times a week.

His

condition slightly got better when he was confined because interventions were started to be done. Because the disease of the client is more on the reproductive organ, the client must be taught to be aware of the assessment of his urine and urination. Usually the normal defecation and urination varies with people so the client must be taught to note any changes in his normal elimination. Pain is also a sign of a problem that must not be ignored by the client and must be reported to his physician.

Exercise

He does not exercise at all because he is busy with work.

He does not perform any type of exercise because he is in complete bed rest.

Individuals who have inactive lifestyles or who are faced with inactivity because of illness or injury are at risk for many problems that can affect major body systems. Clients experience a significant decrease in muscular strength and agility whenever they do not maintain a moderate amount of physical activity. In addition, immobility adversely affects the cardiovascular, respiratory, metabolic, urinary, and psychoneurologic systems. Exercises frequency is about 3 times a week, cumulative 30 minutes daily, and has an intensity of Moderate intensity as measured by the talk test and perceived exertion scale. Interpretation: Working all day in the office does not excuse a person to be not able to exercise. Simple walking when going to the office or using stairs instead of always using the elevator could be a form of exercise to a person working in an office. On the other hand. The client can also exercise even if he is in complete bed rest in the hospital. He could perform range of motion or just a little turning and stretching. It must be a good health teaching for the client that other forms of activities could be an exercise not just going to the gym or jogging in the park. Different alternatives for exercise must be taught to the client so that he can still do exercise. Before he could take a bath every day by himself, change in his own clothes, and do usual hygiene like brushing his teeth, or go to the comfort room by himself. But now he asks his friends to help him change his clothes, after he cleans himself up, assist when brushing his teeth or when he has to go to the bathroom. Analysis: Clients hygienic practices are influenced by numerous factors including culture, religion, environment, developmental level, health and energy, and personal preferences. Hygiene

Sleep and rest

The patient gets up at around 6 pm at night, then goes to work at 8 pm at Eastwood where he works as an IT, and then sleeps at 8 am in the morning. Proper hygiene is very important to be performed by a person. Having diseases or body weakness does not excuse a person from doing proper hygiene. It is good that other people try to help the client perform proper hygiene and he still regards his hygiene as important thing to practice even if he is weak and sick. But while other people are helping him do his hygienic practices, it must be ensured that the client still participates in the best way he can. He must not rely on other people to do it as much as possible especially when he still has the energy to do it. Analysis: Most adults require 8 to 10 hours of sleep each night to prevent undue fatigue and susceptibility to infections. People who are ill require more sleep than normal, and the normal rhythm of sleep and wakefulness is often disturbed. Respiratory conditions can disturb an individuals sleep. Smoking has been associated to be a cause of many diseases including cancer. It may not directly affect his testicular cancer but smoking produces harmful chemicals to the body that may aggravate his illness. The client said before that he still young and wants to live longer so he must follow his doctors and quit smoking and drinking. Peer pressure and following the bandwagon has been a reason why people starts drinking and smoking. His friends can help him to quit drinking and especially smoking.
