FIRST MEETING

NOTES FROM THE CONSULTANT TO THE CONSTRUCTOR

1. DATE OF COMMENTCEMENT 2. SUBMISSIONS I. Performance bond II. CAR policy III. WC policy IV. List of all domestic subcontractors V. Detailed work program with labour histogram VI. Schedule of construction plants VII. Time schedule for submission of samples and approvals VIII. Cash flow statements for contract work incorporating the effect on advance payment, advance recovery, retention etc. . 3. WORK PROGRAMME The contractor is required to submit a detailed work program following all the specifications. The work program should include but is not limited to: - Start date/Project End Date according to the Contract - Time required to mobilize on site - Time allocated for excavation work including sequence of excavation - Time allocated for other major activities - Critical Paths - Construction Sequence - Labor Histograms - Etc. 4. PROJECT STAFF CVs must be submitted of all project employees along with a complete Organization Chart. Senior staff must have previous experience in similar construction. The allocation of Engineers to various tasks must be provided. (Planning/QA-QC Engineer, Finishing Engineer and a safety officer is essential) 5. METHOD STATEMENTS AND SHOP DRAWINGS Do not proceed with any work other than preparation of site huts and removal of top soil before the method statements and shop drawings have been submitted and approved by the Engineer. The method statement should include but is not limited to: - The type of machinery and equipment that will be used - A detailed description with supporting diagrams to show methodology to be adopted - Any calculations to support the procedures adopted (Design Calculations) - If procedures adopted affect the structure (e.g. construction loads on foundations etc.) these must be highlighted in the method statement and

the procedures must be coordinated with the Structural Engineer to ensure that the structure can withstand the construction loads. - Quality assurance procedures that will be adopted - Health and safety procedure - Contingency plan Method statements and shop drawings shall be submitted in three copies and shall be covered with a transmittal sheet. A sample method statement is attached here with. 6. CONSTRUCTION LOGISTICS The Contractor is required to provide all logistical details throughout the construction process including but not limited to: - Material delivery and transportation plan - Loading and unloading bays - Material storage areas, steel bending areas, steel cage fabrication area etc. - Material movement within the site and building. - Permanent equipment movement and storage within the site. - Concrete placement methodology - Office space within the site - Storage areas for samples, test cubes, curing tanks etc. 7. TEST REPORTS The Contractor is required to submit test reports to the Engineer for approval on a timely basis. Payments will not be considered for these items until satisfactory test reports are submitted. Some examples of test reports that will be required throughout the project are listed below: - Steel reinforcement Mill certificate - Steel reinforcement mechanical property to be tested (5 samples from each diameter) - Concrete mix design reports as per the concrete specifications - Cement test reports and certificates - Admixture specifications/Aggregate grading and Specification - Etc. 8. SURVEYING An approved licensed surveyor with experience in similar construction should be responsible to ensure the proper positioning of all components of the structure especially the verticality of the structure throughout construction. The Contractor must submit the methodology they propose to use to ensure verticality of the structure. 9. GENERAL PROCEDURES - Daily progress reports must be submitted (labor and machinery) - Approval for concreting work should be obtained 24 hours before concreting - Sequence of concrete/prop removal to be approved by the Engineer

- Next day program shall be submitted on each day on or before 4 .00 p.m - Weekly program must be submitted to the Resident Engineer (RE) by 9am on Mondays. - Monthly progress reports must be submitted with all relevant photographs - There will be no extra payment for alternate proposals - All communications with the consultant should be through the RE and should be in a Request for Information (RFI) sheet - All verbal instructions shall be confirmed with a verbal order confirmation sheet - Refer to attached sample sheets for formats that need to be followed to record data. 10. ENVIRONMENTAL PLAN & SITE SAFETY All necessary action must be taken during the entire period of construction to ensure that there are no adverse effects on the environment (washing bay, toilet pits etc.). Strict site safety procedures (Safety sign boards, wearing boots and helmets is essential) shall be adopted during the construction to mitigate accidents. A safety Engineer shall be appointed who is responsible for site safety and to maintain site safety according to the specified standards.

11. SITE SECURITY AND ACSESS Site access shall be properly protected with a gate and shall be provided with a washing bay to mitigate movement of mud, concrete etc. to the adjacent road from the site. Main contractors site security huts shall be placed near to the gate and all vehicles and personals shall be inquired at the entrance and site access passes, safety equipments shall be issued after recording the national identity card number. The above mentioned security procedures shall be strictly adopted during Construction period to mitigate undesirable situations and site identification cards shall be issued to all site staff of the main Contractor .
