SUBIC NATIONAL HIGH SCHOOL Subic,Zambales ACCOMPLISHMENT REPORT SY 2009-2010 A. STUDENT DEVELOPMENT 1.

The classroom and club organizations had been organized with 100% success. 2. I-Beryl class had been distributed with books with a ratio of 1:1. 3. 90% out of 264 students in Math I (Beryl, Pearl, Jargon, Topaz & Quartz) had mastered the multiplications tables 1-10. 4. 85% out of 264 students in Math I (Beryl, Pearl, Jargon, Topaz & Quartz) had mastered the desired learning competencies prescribed in the learning areas. 5. The Pre-test, Post Test, DAT and RAT among Math I (3, 6, 8, 16 and 17) class had been administered and evaluated with 95% success. B. TEACHER DEVELOPMENT 1. The teacher attended faculty meetings/conference with 100% success. 2. The teacher had been always present in almost all school activities conducted with full support and participation. C. CURRICULUM DEVELOPMENT AND IMPLEMENTATION 1. Had taught 100% of the learning competencies prescribed in Math I with the aid of well-prepared lesson plan and instructional materials. D. 1. 2. 3. 4. 5. 6. 7. COMMUNITY DEVELOPMENT Participated in the Brigada Eskwela. The Homeroom PTA organization had been organized with 95% success. The teacher had informed the parents of I-Beryl class regarding the rules and regulations as well as the students performance and progress with 95% success. The teacher served as a Room Examiner in the NCAE at Subic National High School last August,2009 and in the National Achievement Test for Second Year (NAT) at Subic National High School last March 11,2010 with 100% success. The teacher actively participate the Review class for the Second Year students in preparation for the National Achievement Test. The teacher served as Committee Member on Canvassing of Mr. & Ms. SNHS 2010 last February 5,2010. The teacher actively participate in the Coronation and Parade of Mr. & Ms. SNHS 2010 as adviser of the 2nd Princess.

E. PHYSICAL FACILITIES DEVELOPMENT 1. The classroom and armchairs of I-Beryl class had been properly equipped with the help of the students. 2. Repainting of walls, chairs and table of I- Beryl class was done.

F. LEGISLATION AND IMPLEMENTATION 1. The wearing of proper uniform among I-Beryl class had been properly implemented. 2. Lesson plans, test results, Form 2 and Form 48 had been passed on the scheduled date. G. RESEARCH AND EVALUATION 1. I-Bery, I-Pearl, I- Jargon, I- Topaz and I- Quartz had been given the pretest in Mathematics I with the MPS of 23.62 %. 2. Periodical Test had been properly administered with the following MPS: 1st Grading 38.1 nd 2 Grading 26.94 3rd Grading 33.32 th 4 Grading 30.4 3. I-Bery, I-Pearl, I- Jargon, I- Topaz and I- Quartz had been given the Post Test in Mathematics I with the MPS of 49.3%, also achieved an MPS of 49.64% in the RAT and 53.92% in the DAT.
