Foreword
The Municipal Ordinance Number C-001 also known as The Code of Administrative Ordinances of the Municipality of Garcia-Hernandez, Bohol, Philippines which was enacted on November 30, 2005 was a landmark legislation which affirmed the Local Government Units commitment towards the improvement of local governance and administration most importantly in the field of public service. On August 2007, almost two years after its adoption, the local executive office formulated a Procedures Manual on Public Service Delivery. The manual includes workflow charts complete with service pledges per office. It was also on the same year that the Environmental Management System [EMS] Manual and Procedures Manual was crafted. The three documents were instrumental not only in the improvement of the work procedures but also in identifying the environmental impacts and aspects that were brought-in from rendering service. The continued implementation of the aforesaid procedures was, indeed, a milestone. Republic Act 9485 also known as the Anti-Red Tape Act of 2007 provides the LGU GarciaHernandez with greater opportunity not only in reviewing the efficiencies of the said procedures but most importantly the creation of a Citizens Charter: a charter that will enhance and improve the service delivery far more faster and wider as well as create a strong capability build-up on the local manpower pool. Initiatives that provide and guide local government capability-building for its effective implementation is an imperative. The skill and the capability of the Local Government Units: the municipality in this behalf which serves primarily as a general purpose government for the coordination and delivery of basic, regular and direct service, in using them into their full advantage, are vital in this said effort. The Citizens Charter as a guidebook will lead the clients via specific procedures on how to avail the service in an approximate time frame as well as identify the responsible persons to be approached to. In this manner the accountability and transparency of the service delivery system of the municipality are measured and determined. The feedback mechanism of the services which is client-based was purposely crafted for the sustainability and improvement of the system. Let us all join together in achieving a more effective, efficient and transparent service delivery.

INDIVIDUALS NEED to secure a Mayor's Clearance before they can apply for a job. People seeking employment are sometimes also required to secure the same. This is especially true of applicants to the Philippine National Police (PNP). Renewal/New Application of Blasters License also requires individuals to submit the same.

A BUILDING Permit is required prior to construction, erection, alteration, major repair, or renovation or conversion of any building/structure owned by government or private entities. Applying for a Marriage License


ABOUT THE SERVICE

WHERE A MARRIAGE license is required, each of the contracting parties shall file separate sworn application for such license with the proper local civil registrar of the place where either or both of the contracting parties reside. The local civil registrar concerned shall enter all applications for marriage license filed with him in a registry book strictly in the order in which the same are received. When the license is issued, the same shall be valid in any part of the Philippines for a period of one hundred twenty (120) days from the date of issue, and shall be deemed automatically cancelled at the expiration of said period if the contracting parties have not made use of it.

Registration of Birth and Marriage Certificates


ABOUT THE SERVICE

REPUBLIC ACT No. 3753 mandates the establishment of a civil register in the Philippines where acts, events, legal instruments and court decrees concerning the civil status of person shall be recorded. The birth of the child, being a vital event of a person, shall be registered within thirty (30) days from the time of birth in the office of the Civil Registrar of the city/municipality where the birth occurred. For ordinary marriage, the time for submission of the Certificate of Marriage is fifteen (15) days following the solemnization of marriage.

Registration of Death Certificates


ABOUT THE SERVICE
IT SHALL BE THE responsibility of the nearest relative or spouse who has knowledge of the death to report the same within forty-eight (48) hours if the deceased died without medical attendance. The health officer shall examine and sign the death certificate and direct the registration of the death certificate to the Office of the Civil Registrar within the reglamentary period of thirty (30) days.

DELAYED REGISTRATION of birth, marriage, death and court decrees like ordinary registration made at the time of the eventshall be filed at the office of the Civil Registrar of the place where the event occurred, following the lapse of the reglamentary period to register.

Requesting endorsement of Registry Records to the Civil Registrar - General


ABOUT THE SERVICE
AS A RULE, all civil registrars shall submit civil registry documents to the Office of the Civil Registrar-General (OCRG) thru their respective NSO provincial offices. There are instances when the NSO cannot issue copy/copies to the interested party because their Office have no available record in its archive, or the current document is still with the NSO provincial office being processed. To facilitate the issuance of requested documents, the concerned Provincial Statistics Officer (PSO) or Civil Registrar is required to submit or indorse the needed document on a piecemeal basis to the NSO.
